摘  要:目的探讨乳腺影像学报告及数据系统(BI-RADS)评分联合超声弹性成像乳腺浸润性癌的鉴别价值。方法回顾性分析我院2021年5月至2022年5月期间收治的乳腺肿块患者108例(108个肿块)BI-RADS评分以及超声弹性成像资料,以病理为金标准,采用χ^(2)检验对比BI-RADS评分、超声弹性成像单独及联合对乳腺浸润性特殊癌鉴别的价值,采用t检验对比良恶性肿块超声弹性评分、BI-RADS评分等参数。结果乳腺恶性肿块共58个,占58/108(53.7%),其中乳头状癌18例、小管癌9例、髓样癌20例、鳞状细胞癌11例;乳腺良性肿块共50个,占50/108(46.3%),其中乳腺纤维腺瘤27例、乳腺叶状肿瘤14例和乳腺腺瘤9例。与BI-RADS评分、超声弹性成像单独检查相比,BI-RADS评分联合超声弹性成像对乳腺浸润性癌诊断敏感度、阳性预测值更高(χ^(2)=4.83,P=0.03;χ^(2)=3.28,P=0.05;χ^(2)=4.49,P=0.03;χ^(2)=2.94,P=0.05);恶性肿块的弹性评分(3.12±0.88)分、应变率比值(3.32±0.72)%、BI-RADS评分(3.46±0.98)分高于良性肿块(1.89±0.44)分、(1.65±0.36)%、(2.03±0.39)分(t=9.24,P<0.001;t=15.34,P<0.001;t=10.06,P<0.001)。结论与BI-RADS评分,超声弹性成像单独检查相比,联合对乳腺浸润性特殊癌鉴别价值更高。Objective To investigate the differential value of breast Imaging Report and Data System(BI-RADS)score combined with ultrasound elastography in breast invasive carcinoma.Methods The BI-RADS scores and ultrasonic elastography data of 108 patients with breast masses admitted to our hospital from May 2021 to May 2022 were analyzed retrospectively χ^(2)To test and compare the value of BI-RADS score and ultrasound elastography alone and in combination in the differential diagnosis of invasive special breast cancer,and to use t test to compare the parameters such as ultrasound elastography score and BI-RADS score of benign and malignant masses.Results There were 58 malignant breast masses,accounting for 58/108(53.70%),including 18 cases of papillary carcinoma,9 cases of tubular carcinoma,20 cases of medullary carcinoma and 11 cases of squamous cell carcinoma.There were 50 benign breast masses,accounting for 50/108(46.30%),including 27 cases of fibroadenoma,14 cases of lobe tumor and 9 cases of adenoma.Compared with BI-RADS score and ultrasonic elastography alone,BI-RADS score combined with ultrasonic elastography has higher diagnostic sensitivity and positive predictive value(χ^(2)=4.83,P=0.028;χ^(2)=3.28,P=0.048;χ^(2)=4.49,P=0.034;χ^(2)=2.94,P=0.048);The elastic score(3.12±0.88),strain rate ratio(3.32±0.72)% and BI-RADS score(3.46±0.98)of malignant tumor were higher than those of benign tumor(1.89±0.44),(1.65±0.36)%and(2.03±0.39)(t=9.24,P<0.001;t=15.34,P<0.001;t=10.06,P<0.001).Conclusion Compared with BI-RADS score and ultrasound elastography alone,the combination is more valuable in differentiating special invasive breast cancer.
