舌侧集中牙合全口义齿修复治疗重度牙槽嵴吸收患者的效果

摘　　要：目的:观察舌侧集中牙合全口义齿修复治疗重度牙槽嵴吸收患者的效果。方法:选取2019年1月至2022年1月该院收治的100例重度牙槽嵴吸收患者进行前瞻性研究,按照随机数字表法将其分为研究组与对照组各50例。对照组采取解剖式牙合型全口义齿修复治疗,研究组采取舌侧集中牙合全口义齿修复治疗,比较两组治疗后咀嚼效率、咬合力、满意度和生命质量[口腔健康影响程度量表(OHIP-14)中文版]评分。结果:研究组佩戴1、3个月的咀嚼效率和咬合力均高于对照组,差异有统计学意义(P<0.05);研究组美观程度、咀嚼功能、固位功能、语音功能和舒适性等满意度评分均高于对照组,差异有统计学意义(P<0.05);佩戴3个月后,两组功能限制、疼痛与不适、能力受限和身心缺陷等OHIP-14评分均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05)。结论:舌侧集中牙合全口义齿修复治疗重度牙槽嵴吸收患者可提高咀嚼效率、咬合力和满意度,改善生命质量,效果优于解剖式牙合型全口义齿修复治疗。Objective:To observe effects of lingual concentrated occlusal complete denture repair in treatment of patients with severe alveolar ridge absorption.Methods:A prospective study was conducted on 100 patients with severe alveolar ridge absorption admitted to the hospital from January 2019 to January 2022.They were divided into study group and control group according to the random number table method,50 cases in each.The control group was treated with anatomical occlusal complete denture repair,while the study group was treated with lingual concentrated occlusal complete denture repair.The masticatory efficiency,the bite force,the satisfaction,and the quality of life[oral health impact profile(OHIP-14)Chinese version]scores were compared between the two groups after the treatment.Results:The masticatory efficiency and the bite force of the study group were higher than those of the control group after wearing for 1 and 3 months,and the differences were statistically significant(P<0.05).The satisfaction scores of aesthetics,masticatory function,retention function,speech function and comfort in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).Further,after 3 months of wearing,the OHIP-14 scores of functional limitation,pain and discomfort,ability limitation and physical and mental defects in the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusions:Lingual concentrated occlusal complete denture repair can improve the masticatory efficiency,the bite force and the satisfaction,and improve the quality of life in the patients with severe alveolar ridge absorption.Moreover,it is superior to anatomical occlusal complete denture repair.
