乌丹降脂胶囊与水飞蓟宾胶囊治疗MAFLD对患者脂代谢和肝功能的效果比较

摘　　要：目的探讨乌丹降脂胶囊和水飞蓟宾胶囊治疗代谢相关脂肪性肝病(MAFLD)对患者脂代谢和肝功能的影响。方法2021年7月-2022年6月我院收治的78例MAFLD患者,通过随机数字表法分为A组和B组,每组39例,分别给予乌丹降脂胶囊、水飞蓟宾胶囊治疗,2组均持续治疗3个月,观察2组治疗前后胰岛素抵抗指数(HOMA-IR)和血脂指标总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)和高密度脂蛋白胆固醇(HDL-C)水平,通过酶联免疫吸附法检测炎性指标核转录因子κB(NF-κB)、转化生长因子-β(TGF-β)、肿瘤坏死因子-α(tumour necrosis factor-α,TNF-α)水平、白介素-6(IL-6)水平,放射免疫沉淀法检测氧化应激指标超氧化物歧化酶(SOD)、丙二醛(MDA)、游离脂肪酸(FFA)水平,测定肝功能指标[丙氨酸转氨酶(ALT)、天门冬氨酸转氨酶(AST)、碱性磷酸酶(ALP)、γ-谷氨酰转移酶(GGT)],比较2组治疗前后B超检查肝脏声像图情况。结果治疗3个月后,2组HOMA-IR水平比较差异无统计学意义(P>0.05);A组血脂TC、LDL-C、TG水平显著低于B组(P<0.05),HDL-C水平显著高于B组(P<0.05);治疗3个月后,A组炎性指标NF-kB、TGF-β、TNF-α、IL-6水平显著低于B组(P<0.05);治疗3个月后,A组氧化应激指标SOD水平显著高于B组(P<0.05),MDA、FFA水平显著低于B组(P<0.05);治疗3个月后,A组肝功能指标ALT、AST、ALP、GGT水平显著低于B组(P<0.05);A组与B组实质回声增强、近场衰减、管道欠清晰、管道纹理消失情况比较,差异无统计学意义(P>0.05)。结论乌丹降脂胶囊可降低MAFLD患者血脂水平,改善机体炎性与氧化应激反应,提高肝功能,效果优于水飞蓟宾胶囊。Objective The aim of this study was to explore the effects of Wudan Lipid-lowering Capsules and Silybin capsules on lipid metabolism and liver function in patients with metabolism-associated fatty liver disease(MAFLD).Methods A total of 78 MAFLD patients admitted to the hospital between July 2021 and June 2022 were recruited.They were randomly divided into group A(39 cases,Wudan Lipid-lowering Capsules)and group B(39 cases,Silybin Capsules).All were treated for 3 months.The homeostasis model assessment-insulin resistance index(HOMA-IR)and blood lipid indexes,including total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),and high density lipoprotein cholesterol(HDL-C)in the two groups were compared before and after treatment.The levels of inflammatory indexes,including the nuclear transcription factorκB(NF-κB),transforming growth factor-β(TGF-β),tumor necrosis factor-α(TNF-α),and interleukin-6(IL-6)were detected by enzyme-linked immunosorbent assay(ELISA).The levels of oxidative stress indexes,including superoxide dismutase(SOD),malondialdehyde(MDA),and free fatty acid(FFA)were detected by radioimmunoprecipitation.Liver function indexes,including alanine aminotransferase(ALT),aspartate aminotransferase(AST),alkaline phosphatase(ALP),and gamma-glutamyltransferase(GGT)were determined.The liver ultrasonographic findings were compared between the two groups before and after treatment.Results After 3 months of treatment,there was no significant difference in HOMA-IR between group A and group B(P>0.05).TC,LDL-C and TG in group A were significantly lower than those in group B,while HDL-C was significantly higher(P<0.05).After 3 months of treatment,inflammatory factors NF-κB,TGF-β,TNF-αand IL-6 in group A were significantly lower than those in group B(P<0.05).After 3 months of treatment,oxidative stress indexes SOD level was significantly higher in group A than that in group B,while MDA and FFA levels were significantly lower than those in group B(P<0.05).After 3 months of treatment,l
