摘 要:目的:利用锥形束CT(CBCT)对安氏Ⅲ类与Ⅰ类错牙合畸形的牙弓与基骨弓宽度之间的协调性进行分析。方法:选取东台市人民医院口腔科就诊的正畸患者60例(年龄18~30岁),其中安氏Ⅲ类与安氏Ⅰ类各30例,收集CBCT扫描数据并进行三维成像,测量上下颌尖牙区、前磨牙区和磨牙区牙弓宽度及相对应的基骨弓宽度,计算同一牙段牙弓宽度与基骨弓宽度之间的差值,比较安氏Ⅲ类与安氏Ⅰ类错牙合畸形牙弓与基骨弓宽度差值的差异。结果:上颌牙弓第一磨牙间牙弓宽度是安氏Ⅲ类大于Ⅰ类(P<0.05),上颌尖牙及第一前磨牙间牙弓宽度比较差异无统计学意义(P>0.05);下颌牙弓尖牙间牙弓宽度及第一磨牙间牙弓宽度均为安氏Ⅲ类大于Ⅰ类(P<0.05),下颌第一前磨牙间牙弓宽度两者差异无统计学意义(P>0.05)。上颌第一前磨牙间基骨弓宽度为安氏Ⅲ类小于Ⅰ类(P<0.05),上颌尖牙间及第一磨牙间基骨弓宽度两者差异无统计学意义(P>0.05);下颌所有牙位基骨弓宽度安氏Ⅲ类大于Ⅰ类(P<0.05)。上颌除尖牙部位以外,牙弓与基骨弓宽度的差值均为安氏Ⅲ类大于Ⅰ类(P<0.05);下颌除第一磨牙部位以外,牙弓与基骨弓宽度的差值为安氏Ⅲ类均小于安氏Ⅰ类(P<0.05)。结论:通过对安氏Ⅲ类与Ⅰ类错牙合畸形的牙弓与基骨弓宽度差异的对比结果分析,在安氏Ⅲ类错牙合畸形的治疗过程中,上颌需增加后牙区基骨宽度来改善其牙弓与基骨弓宽度的协调性,下颌需增加后牙区的牙弓宽度来改善其牙弓与基骨弓宽度的协调性。Objective:To analyze the coordination between the widths of dental and basal bone arches in ClassⅢand ClassⅠmalocclusion using cone⁃beam CT(CBCT).Methods:Sixty patients(18⁃30 y),including 30 cases each of ClassⅢand ClassⅠ,were selected to collect CBCT scans and perform three⁃dimensional imaging to measure the dental arch widths of the maxillary and man⁃dibular cuspid,premolar and molar regions and the corresponding basal bone arch widths.the differences between the dental arch widths in the same tooth sites and the basal bone arch widths were calculated.The difference between the width of the dental arch and the basal bone arch was compared between ClassⅢand ClassⅠmalocclusion.Results:The dental arch width of the first maxillary molar was significantly greater in ClassⅢthan that in ClassⅠ(P<0.05),and there was no difference between the dental arch widths of the maxillary cuspids and the first premolars(P>0.05);the dental arch widths of the mandibular cuspids and the first molars were significantly greater in ClassⅢthan that in ClassⅠ(P<0.05),and there was no difference between the dental arch widths of the mandibular first molars that(P>0.05).The basal bone arch width of the maxillary first premolars was significantly smaller in ClassⅢthan that in ClassⅠ(P<0.05),and the basal bone arch width of the maxillary cuspids and first molars were not different(P>0.05);the basal bone arch widths of all the mandibular tooth sites were significantly greater in ClassⅢthan that in ClassⅠ(P<0.05).In the maxilla,except for the cuspid site,the difference between the dental arch and the basal bone arch widths in ClassⅢwas significantly greater than that in ClassⅠ(P<0.05);in the mandible,except for the first molar site,the difference between the dental arch and the basal bone arch widths in ClassⅢwas significantly smaller than that in ClassⅠ(P<0.05).Conclusions:By comparing the results of the difference between the dental arch and the basal bone arch widths in ClassⅢand ClassⅠmalocclu
