摘　　要：目的:探讨子宫颈胃型腺癌(G-EAC)患者术前、术后血清CA19-9值与临床病理指标及预后的相关性。方法:回顾分析2017年1月至2022年6月空军军医大学第一附属医院收治的24例G-EAC患者术前血清CA19-9值与临床病理特征的关系,绘制受试者工作特征(ROC)曲线,确定对胃型腺癌有评估意义的CA19-9最佳阳性值。随访患者术后血清CA19-9值的变化趋势,分析其与生存状态的相关性。结果:术前血清CA19-9升高与肿瘤的宫颈间质浸润深度密切相关(P<0.01),ROC曲线显示,CA19-9为23U/mL是评估间质浸润深度的最佳界值。术前CA19-9对患者生存状况的影响无统计学意义,术后CA19-9阳性组的预后明显差于正常组(P<0.01)。结论:术前、术后检测G-EAC患者血清CA19-9值可能有助于判断患者肿瘤的浸润程度并监测患者术后的转归状态,为制定临床治疗方案及病情监测提供帮助。Objective:To investigate the correlation between serum CA19-9 level and clinicopathological features and prognosis in patients with gastric-type endocervical adenocarcinoma before and after operation.Methods:A retrospective analysis of 24 patients with gastric-type endocervical adenocarcinoma which admitted to the First Affiliated Hospital of Air Force Military Medical University from January 2017 to June 2022 was conducted.Analyzed the relationship between preoperative serum CA19-9 and clinicopathological features,receiver operating characteristic curve(ROC)of the cases was drawn to evaluate the threshold value of CA19-9 in gastric-type endocervical adenocarcinoma.The postoperative trend of serum CA19-9 was followed up to analyze its correlation with survival status.Results:Preoperative elevation of serum CA19-9 was closely related to the depth of cervical interstitial invasion(P<0.01),the ROC showed that the best cut-off value of CA19-9 for evaluating the depth of interstitial invasion was 23U/mL.Preoperative CA19-9 had no statistically significant effect on the survival status of patients,while postoperative prognosis of CA19-9 positive group was significantly worse than that of normal group(P<0.01).Conclusions:Detection of serum CA19-9 level in patients with gastric-type endocervical adenocarcinoma before and after surgery may be helpful to judge the degree of tumor invasion and monitor the postoperative outcome status of patients,consequently may help for developing clinical treatment plan and monitoring disease.
