摘　　要：目的 :探讨前列腺影像报告和数据系统2.1版(PI-RADS v2.1)联合ADC值对前列腺特异性抗原(PSA)灰区前列腺癌的诊断价值。方法:回顾性收集105例PSA值在4~10μg/L前列腺病变患者的临床及影像资料。所有患者均行3.0 T MRI检查,并于8周内经穿刺或手术得到病理结果,根据病理结果分为前列腺癌组(33例)与非前列腺癌组(72例)。分析PI-RADS v2.1联合ADC值对PSA灰区前列腺癌的诊断价值。结果:PI-RADS v2.1联合ADC值诊断PSA灰区前列腺显著癌的AUC为0.900,高于单独使用PI-RADS v2.1的AUC(0.824),差异有统计学意义(P<0.05)。结论:PI-RADS v2.1联合ADC值可提高对PSA灰区前列腺癌的临床诊断效能。Objective:To explore the evaluation of the Prostate Imaging Reporting and Data System Version 2.1(PI-RADS v2.1)combined with ADC value in the diagnosis of prostate cancer in the PSA gray area.Methods:The clinical and imaging data of 105 patients with PSA of 4~10 ng/mL were retrospectively collected.All patients underwent 3.0 T MRI examination,and pathological results were obtained in 8 weeks.All patients were divided into the prostate cancer group(33 cases)and the non-prostate cancer group(72 cases)according to the pathological results.The diagnostic efficacy of PI-RADS v2.1 combined with ADC value was evaluated by ROC curve analysis.Results:The AUC of PI-RADS v2.1 combined with ADC value in diagnosing prostate cancer in the PSA gray area was 0.900,which was higher than that of PI-RADS v2.1 alone(AUC of 0.824),and the difference was statistically significant(P<0.05).Conclusions:The PI-RADS v2.1 combined with ADC value can improve the diagnostic efficacy for prostate cancer in the PSA gray area.
