依达拉奉联合尼莫地平治疗蛛网膜下腔出血后迟发性脑血管痉挛的效果及对血清S100β蛋白、ET-1水平的影响

摘　　要：目的 分析依达拉奉联合尼莫地平治疗蛛网膜下腔出血(SAH)后迟发性脑血管痉挛(DCV)的效果及对血清S100β蛋白、内皮缩血管肽-1(ET-1)水平的影响。方法 选取2020年6月至2021年6月我院收治的60例SAH后DCV患者为研究对象,依据入院编号奇偶性将其分为对照组与观察组,各30例。对照组给予尼莫地平治疗,观察组在此基础上联合依达拉奉治疗。比较两组的临床疗效、炎症指标、脑动脉血流指标及生化指标。结果 观察组的治疗总有效率显著高于对照组(P<0.05)。治疗后,观察组的白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)及C反应蛋白(CRP)水平均显著低于对照组,白细胞介素-10(IL-10)水平高于对照组(P<0.05)。治疗后,观察组两侧大脑前动脉(ACA)、大脑中动脉(MCA)的收缩期峰值血流速度(PSV)显著低于对照组(P<0.05)。治疗3、7、14、21 d后,观察组的S100β蛋白、ET-1水平均显著低于对照组(P<0.05)。结论 依达拉奉联合尼莫地平治疗SAH后DCV的效果理想,不仅可以减少氧自由基对脑、神经组织的损伤,还可以改善患者的脑血管状态。Objective To analyze the effect of edaravone combined with nimodipine in the treatment of delayed cerebral vasospasm(DCV)after subarachnoid hemorrhage(SAH)and its influences on serum S100βprotein and endothelin-1(ET-1)levels.Methods Sixty patients with DCV after SAH admitted in our hospital from June 2020 to June 2021 were selected as the research objects.According to the parity of admission number,the patients were divided into control group and observation group,with 30 cases in each group.The control group was treated with nimodipine,and the observation group was treated with edaravone on this basis.The clinical efficacy,inflammatory indexes,cerebral artery blood flow indexes and biochemical indexes of the two groups were compared.Results The total effective rate of treatment in the observation group was significantly higher than that in the control group(P<0.05).After treatment,the levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and C-reactive protein(CRP)in the observation group were significantly lower than those in the control group,and the level of interleukin-10(IL-10)was higher than that in the control group(P<0.05).After treatment,the peak systolic velocity(PSV)of anterior cerebral artery(ACA)and middle cerebral artery(MCA)on both sides in the observation group were significantly lower than those in the control group(P<0.05).After 3,7,14 and 21 d of treatment,the levels of S100βprotein and ET-1 in the observation group were significantly lower than those in the control group(P<0.05).Conclusion The edaravone combined with nimodipine in the treatment of DCV after SAH has an ideal effect.It can not only reduce the damage of oxygen free radicals to brain and nerve tissues,but also improve the state of cerebrovascular of patients.
