摘  要:目的:探究B淋巴细胞瘤-2(Bcl-2)、上皮特异抗原(Ber-EP4)、急性淋巴细胞白血病抗原(CD10)、细胞角蛋白20(CK20)和肿瘤细胞增殖指数(Ki-67)免疫组织化学染色在皮肤毛母细胞瘤(TB)与基底细胞癌(BCC)鉴别诊断中的应用效果。方法:选取2020年7月—2021年7月防城港市第一人民医院收集的皮肤TB患者58例及BCC患者40例为观察对象,分析二者bcl-2、ber-EP4、CD10、CK20和Ki-67免疫组织化学染色的表达及差异。结果:BCC患者bcl-2阳性范围>75%多于TB患者,差异有统计学意义(P=0.002);BCC患者bcl-2分布方式多于TB患者,差异有统计学意义(P=0.001)。BCC患者Ber-EP4阳性范围>75%和51%~75%多于TB患者,≤25%少于TB患者,差异有统计学意义(P<0.001);TB与BCC患者Ber-EP4阳性范围26%~50%例数比较,差异有统计学意义(P<0.001)。BBC患者基底样上皮阳性、仅基底样上皮阳性多于TB患者,BCC患者间质阳性少于TB患者,差异有统计学意义(P<0.001)。TB患者CK20阳性多于BCC组,差别有统计学意义(P<0.001)。BCC患者Ki-67阳性指数平均值≥15%多于TB患者,差异有统计学意义(P=0.001);BBC患者Ki-67阳性指数热点区域阳性细胞计数≥25%多于TB患者,差异有统计学意义(P=0.013)。结论:免疫组织化学染色在皮肤TB与BCC的鉴别诊断中应用价值显著。Objective:To investigate the effect of immunohistochemical staining for B lymphoblastoma-2(Bcl-2),epithelial specific antigen(Ber-EP4),acute lymphoblastic leukemia antigen(CD10),cytokeratin 20(CK20),and tumor cell proliferation index(Ki-67)in differential diagnosis of trichoblastoma of the skin(TB)and basal cell carcinoma(BCC).Methods:Fifty-eight cases of cutaneous TB patients and forty BCC specimens collected from Fangchenggang First People's Hospital from July 2020 to July 2021 were selected for observation.The expression and differences of bcl-2,ber-EP4,CD10,CK20 and Ki-67 immunohistochemical staining of the two were analyzed.Results:The bcl-2 positive range of BCC patients was>75%more than that of TB patients,and the difference was statistically significant(P=0.002);BCC patients had more bcl-2 distribution patterns than TB patients,and the difference was statistically significant(P=0.001);Ber-EP4 positive range>75%and 51%-75%in BCC patients were more than TB patients,and≤25%was less than TB patients,with statistically significant difference(P<0.001);Comparison of the number of cases with Ber-EP4 positive range 26%to 50%between TB and BCC patients,the difference was statistically significant(P<0.001);More basal-like epithelium positivity and basal-like epithelium positivity only in BBC patients than in TB patients,and less interstitial positivity in BCC patients than in TB patients,with a statistically significant difference(P<0.001);TB patients had more CK20 positivity than BCC group,and the difference was statistically significant(P<0.001);BCC patients had more Ki-67 positive index mean≥15%than TB patients,and the difference was statistically significant(P=0.001);BBC patients had≥25%more positive cell counts in the hotspot area of Ki-67 positivity index than TB patients,and the difference was statistically significant(P=0.013).Conclusion:The application value of immunohistochemical staining in the differential diagnosis of cutaneous TB and BCC is significant.
