摘 要:目的检测宫颈病变患者血清卵泡抑素样蛋白1(FSTL1)、转化生长因子-β1(TGF-β1)水平及高危型人乳头瘤病毒(HR-HPV)载量,探讨三者联合检测对宫颈病变的预测价值。方法选取2019年4月—2022年6月于本院接受治疗的164例宫颈病变患者为研究对象,根据病理检查结果将其分为宫颈上皮内瘤变(CIN)Ⅰ级患者82例、CINⅡ级患者36例、CINⅢ级患者24例及宫颈癌患者22例。采用酶联免疫吸附(ELISA)法检测血清FSTL1、TGF-β1水平;采用第二代杂交捕获法检测HR-HPV载量;分析HR-HPV载量、血清FSTL1、TGF-β1及三者联合检测对CINⅠ级、CINⅡ级、CINⅢ级及宫颈癌诊断的阳性符合率,并采用卡方检验比较其差异。结果CINⅠ级、CINⅡ级、CINⅢ级及宫颈癌患者年龄、婚姻状况、初次性生活年龄、文化程度、孕次、产次比较,差异无统计学意义(P>0.05)。以病理结果作为金标准,HR-HPV载量、血清FSTL1、TGF-β1及三者联合检测对CINⅠ级、CINⅡ级、CINⅢ级及宫颈癌诊断的阳性符合率分别为77.44%(127/164)、64.63%(106/164)、63.41%(104/164)、86.59%(142/164)。HR-HPV载量、血清FSTL1、TGF-β1联合检测对CINⅠ级、CINⅡ级、CINⅢ级及宫颈癌诊断的阳性符合率高于三者单独检测,差异有统计学意义(P<0.05)。结论HR-HPV载量及血清中FSTL1、TGF-β1水平与宫颈病变的发生密切相关,三项指标联合检测可预测宫颈病变。Objective This paper aims to detect the serum levels of follistatin like protein-1(FSTL1),transforming growth factor-β1(TGF-β1)and high-risk human papillomavirus(HR-HPV)in patients with cervical lesions,and to explore the predictive value of the three combined detection for cervical lesions.Methods A total of 164 patients with cervical lesions who were treated in our hospital from April 2019 to June 2022 were selected as the research objects,according to the results of pathological examination,they were divided into 82 cases of cervical intraepithelial neoplasia(CIN)Ⅰ,36 cases of CINⅡ,24 cases of CINⅢ and 22 cases of cervical cancer.The serum levels of FSTL1 and TGF-β1 were detected by enzyme-linked immunosorbent assay(ELISA);HR-HPV load was detected by the second generation hybridization capture method;the positive coincidence rate of HR-HPV load,serum FSTL1,TGF-β1 and combined detection in the diagnosis of CINⅠ,CINⅡ,CINⅢ and cervical cancer was analyzed.Chi square test was used to compare the differences.Results There was no statistical significance on the differences in age,marital status,age of first sexual life,educational level,pregnancy times and delivery times among CINⅠ,CINⅡ,CINⅢ and cervical cancer patients(P>0.05).Taking pathological results as gold standard,the positive coincidence rate of HR-HPV load,serum FSTL1,TGF-β1 and their combined detection for CINⅠ,CINⅡ,CINⅢ and cervical cancer was 77.44%(127/164),64.63%(106/164),63.41%(104/164),86.59%(142/164),respectively.The positive coincidence rate of combined detection of HR-HPV load,serum FSTL1 and TGF-β1 for CINⅠ,CINⅡ,CINⅢ and cervical cancer was higher than that of the three alone,and the differences were statistically significant(P<0.05).Conclusion HR-HPV load and the serum levels of FSTL1 and TGF-β1 are closely related to the occurrence of cervical lesions.The combined detection of the three indicators can predict cervical lesions.
