中医湿、瘴、疠邪源流合考

摘　　要：湿邪、瘴气、疠气是中医病因理论中三种外感邪气。湿邪属六淫之一,理论发源最早,瘴气、疠气是后世医家在六淫之外另立的特殊邪气。现代中医外感病因分为六淫和疠气两类,瘴气往往被归属于疠气,但在历史源流中,瘴气是瘟疫病因从六淫说向疠气说发展的中介。瘴气是岭南地域环境下的湿邪,疠气是突破地域限制的瘴气,吴又可所述瘟疫症状与瘴疟接近,其达原饮是由前人治瘴方常山散化裁而来。疠气因传染性、致病特异性而立名,但与“因发知受”的六淫并不对立,吴又可所面对的瘟疫应属湿疫。湿、瘴、疠邪定义角度不同,其源流相互交叉,汇通三者治法对现今瘟疫治疗仍有启示意义。The dampness,Zhang and Li Qi are three types of exogenous pathogens in the etiology theory of Traditional Chinese Medicine.Dampness is one of the six evils and has the earliest theory origin.The Zhang and Li Qi are special evils proposed by later physicians in addition to the six evils.The pathogenesis of exogenous diseases in modern Chinese Medicine is divided into two types:the six evils and the Li Qi.The Zhang Qi is often attributed to Li Qi.However,in the historical origin,the Zhang Qi is the mediator for the development of the pathogenesis of plague from the six evils to the Li Qi.The Zhang Qi refers to a kind of dampness evil in the Lingnan area,and the Li Qi is the Zhang Qi that breaks through geographical restrictions.The symptoms of the plague described by Wu Youke are similar to those of malaria,and the Dayuan decoction is derived from Changshan decoction,a former prescription for Zhang Qi.The Li Qi is named for its contagiousness and the specificity of pathogenicity,but it is not opposed to the six evils,which are distinguished by symptoms.The plague that Wu Youke faced should be a damp plague.The dampness,Zhang,and Li Qi are defined from different angles,but their sources and streams cross each other.The combined textual research of the dampness,Zhang and Li Qi will have enlightening significance for the treatment of plague.
