颅内恶性胶质瘤术后替莫唑胺单纯化疗联合放疗的疗效观察

摘　　要：目的探讨替莫唑胺单纯化疗联合放疗对颅内恶性胶质瘤术后患者的疗效。方法选取2019年1月~2022年12月符合条件的颅内恶性胶质瘤患者76例,按照随机数字法分为对照组和观察组,两组患者均采用手术治疗,其中对照组术后采用替莫唑胺单纯化疗治疗,观察组采用替莫唑胺化疗联合放疗治疗。评估两组患者临床疗效、生存率、中位生存时间和中位复发时间、不良反应及生活质量。结果治疗后观察组的CBR、ORR及KPS评分均明显高于对照组(P<0.05);治疗后1年、2年、3年两组患者的生存率呈下降趋势,但观察组的生存率明显高于对照组(P<0.05);观察组的中位生存时间和中位复发时间均长于对照组(P<0.05);两组患者的胃肠反应、粒细胞减少、贫血、骨髓抑制及肝肾毒性等不良反应差异无统计学意义(P>0.05)。结论替莫唑胺化疗联合放疗较单纯化疗用于颅内恶性胶质瘤术后可提高患者的治疗效果,延长患者的生存期,且未明显增加不良反应,提升患者的生活质量。Objective To investigate the effect of temozolomide combined with radiotherapy on postoperative patients with intracranial malignant glioma.Methods 76 patients with intracranial malignant glioma who met the conditions in our hospital from January 2019 to December 2022 were randomly divided into the control group and the observation group.The patients in the two groups were treated by surgery,in which the control group was treated with temozolomide alone after surgery,and the observation group was treated with temozolomide chemotherapy combined with radiotherapy.The clinical efficacy,survival rate,median survival time,median recurrence time,adverse reactions and quality of life of the two groups were evaluated.Results After treatment,the scores of CBR,ORR and KPS in the observation group were significantly higher than those in the control group(P<0.05);The survival rate of patients in the two groups showed a downward trend at 1,2 and 3 years after treatment,but the survival rate in the observation group was significantly higher than that in the control group(P<0.05);The median survival time and median recurrence time of the observation group were longer than those of the control group(P<0.05);There was no significant difference in adverse reactions such as gastrointestinal reaction,granulocytopenia,anemia,bone marrow suppression and hepatorenal toxicity between the two groups(P>0.05).Conclusion Chemotherapy with temozolomide combined with radiotherapy can improve the treatment effect and prolong the survival period of patients with intracranial malignant glioma after surgery,without significantly increasing adverse reactions and improving the quality of life of patients.
