摘 要:目的:探讨术前运动训练对老年肺癌患者围手术期心肺功能及并发症的影响。方法:选取2020年9月—2022年11月中山市人民医院胸外科收治的60例老年肺癌患者,按照随机数字表法分成康复组(n=30)和对照组(n=30)。两组患者在术前准备相同、实施呼吸训练的情况下,康复组患者术前接受有氧运动训练,对比两组术前心肺功能及术后并发症等指标。结果:手术前1 d,两组呼气流量峰值(PEF)、最大摄氧量(VO_(2)max)较入院时均提高,康复组均高于对照组,差异均有统计学意义(P<0.05)。两组术后第1天咳嗽能力等级均下降,康复组优于对照组,差异均有统计学意义(P<0.05)。出院前1 d康复组6MWT比对照组距离更长,差异有统计学意义(P<0.05)。康复组术后住院天数短于对照组,术后并发症总发生率低于对照组,差异均有统计学意义(P<0.05)。结论:术前运动训练可以提高老年肺癌患者术前的心肺功能,减少术后并发症。Objective:To explore the effect of preoperative exercise training on perioperative cardiopulmonary function and complications in elderly patients after lung cancer surgery.Method:From September 2020 to November 2022,60 elderly patients with lung cancer of Thoraic Surgery Department of Zhongshan City People's Hospital were selected,they were divided into rehabilitation group(n=30)and control group(n=30)according to random number table method.Both groups received routine preoperative preparation and respiratory training,and the rehabilitation group received aerobic exercise.The differences of preperative cardiopulmonary function and the incidence of postoperative complications were compared between the two groups.Result:1 day before operation,PEF and VO_(2)max in both groups were higher than those at admission,and the rehabilitation group were higher than those of the control group,the differences were statistically significant(P<0.05).The grade of coughing ability of both groups decreased on the first day after operation,and the rehabilitation group was better than that of the control group,the differences were statistically significant(P<0.05).1 day before discharge,the distance of 6MWT in the rehabilitation group was longer than that in the control group,the difference was statistically significant(P<0.05).The postoperative hospitalization days in the rehabilitation group was shorter than that in the control group,and the total incidence of postoperative complications was lower than that in the control group,the differences were statistically significant(P<0.05).Conclusion:Preoperative aerobic exercise training can improve the cardiopulmonary function and reduce postoperative complications in elderly patients with lung cancer.
