摘 要:目的探讨ADAM9和p53在卵巢癌中的潜在诊断和预后价值。方法选取2016年01月至2022年12月间泰州市人民医院收治的128例卵巢癌患者,并收集患者的肿瘤组织和癌旁组织,对不同患者根据ADAM9和p53的表达水平进行临床病理特征与生存周期比较,分析ADAM9和p53的表达与卵巢癌病理特征及预后的相关性。结果ADAM9在卵巢肿瘤组织中的表达水平高于癌旁组织(χ^(2)=5.082,P<0.05),p53在卵巢肿瘤组织中的表达水平低于癌旁组织(χ^(2)=12.623,P<0.001)。RT-PCR的检测发现ADAM9在卵巢肿瘤组织中mRNA高表达,p53在卵巢组织中mRNA低表达;并且ADAM9和p53二者之间具有显著相关性,呈负相关(P<0.0001,r=-0.123)。ROC曲线分析显示ADAM9和p53的AUC值分别为0.924(95%置信区间为0.880~0.967)和0.851(95%置信区间为0.796-0.906),表示ADAM9和p53是诊断卵巢癌的敏感标志物。ADAM9高表达组和p53低表达组卵巢肿瘤患者的无病进展生存期(PFS)均明显降低。结论卵巢癌发生发展过程与ADAM9和p53的表达异常有关,可作为临床诊断和预后的标志物。Objective To investigate the expression of p53 and ADAM9 in ovarian epithelial carcinoma and its rela⁃tionship with potential diagnostic and prognostic value.Methods From January 2016 to December 2020,90 cases of ovarian tumor patients from Taizhou People's Hospital,and the clinicopathological characteristics and survival status of different patients were compared according to the expression of ADAM9 and p53.The pathological characteristics and prognosis of ovarian cancer were correlated between the expression of ADAM9 and p53.Re⁃sults The expression level of ADAM9 in ovarian cancer is much higher than that in para-carcinoma tissue(x^(2)=5.689,P<0.05),the expression level of p53 in ovarian cancer is much lower than that in para-carcinoma tissue(x^(2)=11.310,P<0.001).In fresh tumor samples,the mRNA expression of ADAM9 was increased and the mRNA ex⁃pression of p53 was decreased compared with the normal tissues by RT-PCR methods(P<0.05).There was a significant correlation between ADAM9 and p53,which was positively correlated with each other(P<0.0001,r=-0.123).The results of ROC analysis suggested that the AUC of ADAM9 and p53 for ovarian cancer were 0.924(95%CI 0.880~0.967)and 0.851(95%CI 0.796-0.906),indicating that ADAM9 and p53 are sensitive markers for the diagnosis of ovarian cancer.The high expression of ADAM9 and the low expression of p53 may indicate decreased Progression-Free Survival of the patients.Conclusion The development of ovarian cancer is related to the abnormal expression of ADAM9 and p53,and they may serve as potential diagnostic and prognos⁃tic markers for the diagnosis and treatment of ovarian cancer.
