大便潜血试验联合血清CEA、 CA199检测对结直肠癌术后复发的预测价值研究

摘　　要：目的:研究大便潜血试验和肿瘤标志物血清癌胚抗原(CEA)、糖类抗原199(CA199)检测对结直肠癌根治术后复发的预测价值,为临床提高结直肠癌患者预后和降低结直肠癌患者复发率提供依据。方法:选取2020年1月至2022年12月收治的经病理检查确诊为结直肠癌患者110例,将其随机分为对照组和观察组,每组55例。对照组患者单纯行血清CEA、CA199检测,观察组患者行大便潜血试验联合血清CEA、CA199检测,术后1周对患者进行检测,比较2组患者的阳性率并对2组患者随访1年,观察患者的复发率以及灵敏度、特异度和准确度。结果:观察组患者复发阳性率高于对照组(P<0.05),2组患者的特异度比较差异无统计学意义(P>0.05),观察组患者的灵敏度、准确度均高于对照组(P<0.05)。结论:大便潜血试验联合血清CEA、CA199检测能有效预测结直肠癌患者术后复发,且具有良好的灵敏度和准确度。Objective To investigate the predictive value of occult blood test of stool(OBTS)combined with detection of serum CEA and CA199 on postoperative recurrence of colorectal cancer,to provide a clini-cal basis for improving the prognosis of colorectal cancer patients and reducing the recurrence rate of color-ectal cancer patients.Methods Randomly divided 110 colorectal cancer patients treated in author's hospi-tal(2020-01-2022-12)and inspected as confirmed cases pathologically into control group(55 cases,inspec-ted by alone detection of CEA and CA199)and observation group(55 cases,by above combination of detec-tion,i.e,added OBTS),the 1lo cases were all inspected at 1 week after surgery;then,compared both groups'the ratio of patients being positive;the recurrence rate,as well as sensitivity,specificity,and accura-cy shown by followed-up of 1 year.ResultsThe positive rate of recurrnce in observation group was higher than that in control group(P<0.05);in specificity there was no statistical difference between the two groups(P>0.05);in sensitivity and accuracy observation group was all higher than control group(all,P<0.05).Conclusion After radical surgery for colorectal cancer OBTS combined with detection of serum CEA and CA199 can effectively prognose patients'recurrence,and with better sensitivity and ac-curacy.
