摘 要:目的制备新型栓塞剂——超顺磁性碘油(SPLIP),观察其用于经导管动脉栓塞(TAE)治疗兔VX2肝癌模型的安全性、有效性及MRI所示TAE栓塞肿瘤效果。方法制备新型栓塞剂SPLIP,观察其稳定性,比较制备当日与第30天MRI所示体外SPLIP T1和T2值。分别将10只健康雄性新西兰大白兔随机分为SPLIP组和常规碘油(C-LIP)组(每组5只)、10只雄性新西兰大白兔VX2肝癌模型随机分为SPLIP-A组和C-LIP-A组(每组5只),行TAE,对SPLIP组和SPLIP-A组注入0.6 ml SPLIP,C-LIP组和C-LIP-A组注入等量C-LIP,2周后对SPLIP-A组和C-LIP-A组行上腹部平扫CT及MRI,观察肿瘤影像学表现,评估SPLIP用于TAE治疗VX2肝癌模型兔效果;对比各组兔TAE前及2周后血清谷丙转氨酶(GPT)、谷草转氨酶(GOT)、血尿素氮(BUN)及肌酐(Cr)变化。结果所获SPLIP性质稳定;体外MR T1WI和T2WI均无信号,制备当日与第30天T1及T2信号值差异均无统计学意义(P均>0.05)。治疗后2周,SPLIP组与C-LIP组GPT、GOT、BUN及Cr均与治疗前差异无统计学意义(P均>0.05);MRI所示SPLIP-A组及C-LIP-A组碘油沉积范围及栓塞肿瘤效果与CT相符。结论SPLIP用于TAE治疗兔VX2肝癌模型安全、有效;利用MRI可清晰显示其栓塞肿瘤效果。Objective To prepare a novel embolization agent superparamagnetic lipiodol(SPLIP),and to observe its safety and effectiveness in transcatheter arterial embolization(TAE)for rabbit VX2 hepatocarcinoma model,as well as embolic effect of TAE showed on MRI.Methods SPLIP was prepared,its stability was observed,while T1 and T2 values of SPLIP on MRI on the preparation day and 30 days after preparation were compared.Ten healthy male New Zealand white rabbits were randomly divided into SPLIP group and conventional lipiodol(C-LIP)group(each n=5),while 10 male New Zealand white rabbit models of VX2 hepatocarcinoma were randomly divided into SPLIP-A group and C-LIP-A group(each n=5).TAE was performed,during which rabbits in SPLIP group and SPLIP-A were injected with 0.6 ml SPLIP,in C-LIP group and C-LIP-A group were injected with the same amount of C-LIP,respectively.Serum glutamic-pyruvic transaminase(GPT),glutamic-oxaloacetic transaminase(GOT),blood urea nitrogen(BUN)and creatinine(Cr)were observed before and 2 weeks after TAE.Upper abdominal CT and MRI were performed 2 weeks after TAE in SPLIP-A group and C-LIP-A group,and the efficacy of SPLIP for TAE of VX2 hepatocarcinoma rabbit models was evaluated.Results The physical properties of SPLIP were stable,and no signal was found on MR T1WI nor T2WI.There was no significant difference of T1 nor T2 signal values on the preparation day and 30 days after preparation(both P>0.05).Two weeks after TAE,no significant difference of serum GPT,GOT,BUN nor Cr was found in SPLIP group and C-LIP group compared with those before TAE(all P>0.05).The extent of embolic agent deposition and embolic effect of tumors showed on MRI in SPLIP-A and C-LIP-A groups were consistent with those on CT.Conclusion SPLIP was safe and effective for TAE of VX2 hepatocarcinoma rabbit models,and the embolic effect of tumors could be clearly displayed on MRI.
