摘  要:目的观察^(18)F-FDG PET/CT代谢参数联合Ki-67指数鉴别胃肠胰(GEP)神经内分泌瘤(NET)G3与GEP神经内分泌癌(NEC)的价值。方法回顾性分析接受^(18)F-FDG PET/CT检查的41例GEP-神经内分泌肿瘤(NEN)患者,根据WHO 2019分类标准将其分为NET G3组(n=19)和NEC组(n=22);比较组间临床、病理资料及PET/CT代谢参数差异,构建logistic模型,分析PET/CT参数联合Ki-67指数鉴别诊断NET G3与NEC的价值。结果NET G3组9例肿瘤位于胃肠、10例位于胰腺,NEC组17例肿瘤位于胃肠、5例位于胰腺。NET G3组与NEC组肿瘤原发部位、Ki-67指数、肝转移情况、最大标准摄取值(SUV_(max))、峰值标准摄取值(SUV_(peak))、肿瘤-肝脏比值(TLR)及肿瘤-血池比值(TBR)差异均有统计学意义(P均<0.05)。Ki-67、肿瘤SUV_(max)、肿瘤SUV_(peak)、TLR和TBR鉴别诊断NET G3与NEC的曲线下面积(AUC)分别为0.949、0.699、0.706、0.718和0.691,Ki-67+肿瘤SUV_(max)、Ki-67+肿瘤SUV_(peak)、Ki-67+TLR和Ki-67+TBR鉴别NET G3与NEC的AUC分别为0.952、0.952、0.957和0.955。结论^(18)F-FDG PET/CT代谢参数联合Ki-67指数能有效鉴别GEP-NET G3与GEP-NEC。Objective To observe the value of^(18)F-FDG PET/CT metabolic parameters combined with Ki-67 index for differential diagnosis of gastroenteropancreatic(GEP)-neuroendocrine tumor(NET)G3 and GEP-neuroendocrine carcinoma(NEC).Methods Data of 41 patients with GEP-neuroendocrine neoplasm(NEN)were retrospectively analyzed.The patients were divided into NET G3 group(n=19)or NEC group(n=22)according to WHO 2019 classification criteria.Clinical,pathological data and PET/CT metabolic parameters were compared between groups,logistic models were constructed,and the value of PET/CT parameters combined with Ki-67 index for differential diagnosis of NET G3 and NEC was analyzed.Results There were 27 cases in NET G3 group and 22 cases in NEC group,9 cases with gastrointestinal tumor and 10 pancreatic tumor in the former,while 17 with gastrointestinal tumor and 5 with pancreatic tumor in the latter.Significant differences of tumor primary site,Ki-67 index,liver metastasis,the_(max)imum standard uptake value(SUV_(max)),peak standard uptake value(SUV_(peak)),ratio of tumor SUV_(max)to liver SUV mean(TLR)and tumor SUV_(max)to blood SUV mean(TBR)were found between groups(all P<0.05).The area under the curve(AUC)of Ki-67,tumor SUV_(max),tumor SUV_(peak),TLR and TBR for differential diagnosis of NET G3 and NEC was 0.949,0.699,0.706,0.7^(18)and 0.691,respectively,of Ki-67+tumor SUV_(max),Ki-67+tumor SUV_(peak),Ki-67+TLR and Ki-67+TBR was 0.952,0.952,0.957 and 0.955,respectively.Conclusion^(18)F-FDG PET/CT metabolic parameters combined with Ki-67 index was valuable for differentiating GEP-NET G3 and GEP-NEC.
