摘　　要：目的探究CalliSpheres载药微球与普通微球在局部晚期宫颈癌化疗栓塞中的应用及预后。方法选取2017年7月至2020年8月在鄂尔多斯市中心医院就诊并确诊为局部晚期宫颈癌的患者80例,通过随机数字表法分为治疗组41例,予阿霉素载药微球行子宫动脉栓塞化疗,对照组39例,予普通微球子宫动脉栓塞^(+)阿霉素子宫动脉灌注化疗,两组均治疗2个周期。对比两组患者的临床疗效、肿瘤标志物、免疫功能的改善情况,评估治疗后不良反应及生存期。结果治疗组总缓解率明显高于对照组(P<0.05);治疗后,治疗组血清肿瘤标志物(SCC-Ag、CEA、CA199、CA125)均低于对照组(P<0.05);治疗后,对照组CD4^(+)、CD4^(+)/CD8^(+)均低于治疗组(P<0.05),CD8^(+)高于治疗组(P<0.05)。两组患者2年的总生存率差异无统计学意义(P>0.05)。治疗后,治疗组不良反应发生率及严重程度均低于对照组(P<0.05)。结论CalliSpheres载药微球较普通微球可更有助于提高化疗栓塞治疗宫颈癌的疗效,尽管改善预后的效果有限,但能抑制血清肿瘤标志物表达并改善患者的免疫功能,对预防不良反应及减轻其病情严重程度有利。Objective To explore the application and prognosis of CalliSpheres drug-loaded microspheres and ordinary microspheres in chemoembolization of locally advanced cervical cancer.Methods 80 patients who were treated between July 2017 and August 2020 and diagnosed as locally advanced cervical cancer in Ordos Central Hospital were selected and divided into treatment group(n=41,adriamycin-loaded microspheres for uterine artery chemoembolization)and control group(n=39,ordinary microsphere uterine artery embolization^(+)adriamycin uterine artery infusion chemotherapy)by the random number table method.Both groups were treated for 2 cycles.The clinical efficacy,tumor markers,immune function improvement of the two groups were compared,and the adverse reactions and survival time after treatment were evaluated.Results The total remission rate in treatment group was significantly higher than that in control group(P<0.05).After treatment,the levels of serum tumor markers(SCC-Ag,CEA,CA199,CA125)were all lower in treatment group than those in control group(P<0.05).The CD4^(+)and CD4^(+)/CD8^(+)in treatment group were both higher than those in control group(P<0.05)while the CD8^(+)was lower(P<0.05).There was no statistical difference in 2-year overall survival rate between the two groups(P>0.05).After treatment,the incidence rates and severity of adverse reactions in treatment group were lower than those in control group(P<0.05).Conclusions Compared with ordinary microspheres,CalliSpheres drug-loaded microspheres can help to enhance the efficacy of chemoembolization in the treatment of cervical cancer.Callispheres can inhibit the expressions of serum tumor markers and improve the immune function,and it is beneficial to preventing the adverse reactions and relieving the severity to a limited extent.
