摘　　要：目的探讨经尿道膀胱肿瘤铥激光切除术(TmLRBT)应用于非肌层浸润性膀胱癌(NMIBC)的临床效果及对炎症因子、生活质量的影响。方法选择2019年6月至2020年11月在本院就诊的106例NMIBC患者,按照随机数字表法分为观察组和对照组,每组各53例。对照组行经尿道膀胱肿瘤钬激光切除术(HoLRBT),观察组行TmLRBT。对比两组手术相关指标、术前、术后6个月的炎症指标[中性粒细胞与淋巴细胞比值(NLR)、血小板与淋巴细胞比值(PLR)、免疫炎症指数(SII)]、生活质量评分、术后并发症发生率。结果两组NMIBC患者的手术时间、膀胱刺激征发生率、术前NLR、PLR、SII、生活质量评分比较,差异均无统计学意义(均P>0.05);观察组的术中出血量、术后NLR、PLR、SII低于对照组,差异均有统计学意义(均P<0.05)。观察组的术后冲洗时间、尿管留置时间及住院总时间短于对照组,术后生活质量评分高于对照组(均P=0.001)。结论TmLRBT应用于NMIBC的临床效果显著,可有效改善炎症因子及生活质量,优化手术相关指标,值得临床推广。Objective To explore the clinical effect of transurethral thulium laser resection of bladder tumor(TmLRBT)in non-muscular invasive bladder cancer(NMIBC)and its influence on inflammatory factors and quality of life.Methods A total of 106 NMIBC patients treated in our hospital from June 2019 to November 2020 were selected and divided into observation group and control group according to random number table method,with 53 cases in each group.The control group received transurethral holmium laser resection of bladder tumor(HoLRBT),and the observation group received TmLRBT.Preoperative and postoperative inflammatory indicators[neutrophil to lymphocyte ratio(NLR),platelet to lymphocyte ratio(PLR),immunoinflammatory index(SII)],quality of life score,and postoperative complication rate were compared between the two groups.Results There were no significant differences in operation time,incidence of bladder irritation,preoperative NLR,PLR,SII and quality of life scores between the two groups(all P>0.05).The intraoperative blood loss,NLR,PLR and SII in the observation group were lower than those in the control group,and the differences were statistically significant(all P<0.05).Postoperative irrigation time,catheter retention time and total hospital stay in the observation group were shorter than control group,and postoper ative quality of life score was higher than control group(all P=0.001).Conclusions The application of TmLRBT in NMIBC has significant clinical effect,which can effectively improve inflammatory factors and quality of life,optimize surgery-related indicators,and is worthy of clinical promotion.
