清胃汤联合甲硝唑片治疗牙周炎疗效观察及对炎症因子、牙周健康状况的影响

摘　　要：目的:观察自拟清胃汤联合甲硝唑片治疗牙周炎的临床疗效及对牙周健康状态的改善效果。方法:选取牙周炎患者104例,按随机数字表法分为治疗组与对照组各52例。2组均给予常规处理,对照组给予甲硝唑片治疗,治疗组在对照组基础上给予清胃汤治疗。比较2组临床疗效、牙周健康状况、龈沟液炎症因子[白细胞介素-6 (IL-6)、白细胞介素-1β (IL-1β)及肿瘤坏死因子-α (TNF-α)]水平及不良反应发生率。结果:治疗组总有效率为96.15%,对照组为82.69%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组牙龈沟出血指数、牙龈指数及菌斑指数评分均较治疗前降低(P<0.05),且治疗组上述3项评分均低于对照组(P<0.05)。治疗后,2组IL-6、IL-1β、TNF-α水平均较治疗前降低(P<0.05),且治疗组上述各项指标均低于对照组(P<0.05)。治疗组不良反应发生率为9.62%,对照组为7.69%,2组比较,差异无统计学意义(P>0.05)。结论:清胃汤联合甲硝唑片治疗牙周炎疗效显著,可有效减轻局部炎症反应,有利于改善患者牙周健康状况,且用药安全性良好。Objective:To observe the clinical effect of self-made Qingwei Decoction combined with Metronidazole Tablets for periodontitis and their improvement effect on inflammatory factors and periodontal health.Methods:A total of 104 cases of patients with periodontitis were selected and divided into the treatment group and the control group according to the random number table method,with 52 cases in each group.Both groups were given routine treatment;the control group was treated with Metronidazole Tablets,and the treatment group was additionally treated with self-made Qingwei Decoction based on the treatment of the control group.Clinical effects,periodontal health,the levels of inflammatory factors in gingival crevicular fluid[interleukin-6(IL-6),interleukin-1β(IL-1β)and tumor necrosis factor-α(TNF-α)],and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate was 96.15%in the treatment group and 82.69%in the control group,the difference being significant(P<0.05).After treatment,the scores of gingival sulcus bleeding indexes and the gingival and plaque indexes in the two groups were decreased when compared with those before treatment(P<0.05),and the above three scores in the treatment group were lower than those in the control group(P<0.05).After treatment,the levels of IL-6,IL-1β,and TNF-αin the two groups were decreased when compared with those before treatment(P<0.05),and the above three levels in the treatment group were lower than those in the control group(P<0.05).The incidence of adverse reactions was 9.62%in the treatment group and 7.69%in the control group,there being no significance in the difference(P>0.05).Conclusion:Qingwei Decoction combined with Metronidazole Tablets has a significant curative effect in the treatment of periodontitis,which can effectively relieve local inflammatory responses and help to improve their periodontal health,with good medication safety.
