摘 要:目的探讨传动直丝弓技术非减数矫治成人骨性Ⅱ类非高角错[牙合]畸形的临床疗效。方法筛选2017年1月至2020年1月于华中科技大学同济医学院附属协和医院口腔医学中心就诊,确诊为成人骨性Ⅱ类非高角错[牙合]畸形,采用传动直丝弓技术非减数矫治完成的患者15例(男性6例,女性9例,平均年龄25.7岁),对治疗前后头颅侧位片和锥形束CT影像资料测量分析得出数据进行回顾性研究。结果患者平均疗程18.3个月,矫治后前牙覆覆盖恢复正常,尖牙和磨牙关系达到中性。治疗后SNA角减小(1.2±1.0)°(P=0.001),ANB角减小(1.3±0.9)°(P<0.001),上颌中切牙压低(2.4±1.4)mm(P<0.001),下颌中切牙压低(3.1±1.2)mm(P<0.001),上颌第一磨牙远中移动(3.5±0.9)mm(P<0.001)。无1例患者出现骨开裂/骨开窗等并发症。治疗后发生2 mm以上牙根吸收的上下切牙占所有切牙总数4.16%。结论传动直丝弓矫治技术对成人骨性Ⅱ类错[牙合]畸形非减数矫治取得良好的效果,牙根吸收和骨开裂/骨开窗等正畸并发症发生率极低。Objective To investigate the clinical effect of non-extraction treatment of adult non-high-angle skeletal ClassⅡmalocclusion with transmission straight wire technique.Methods Fifteen adult patients(6 males,9 females,25.7 years old on average)who were diagnosed with skeletal ClassⅡmalocclusion and completed non-extraction orthodontic treatment at the Department of Stomatology,Union Hospital,Tongji Medical College,Huazhong University of Science and Technology from January 2017 to January 2020 were selected.A retrospective study was conducted on their lateral cephalograms and cone-beam CT data before and after treatment.Results The average course of treatment for all patients was 18.3 months.After treatment,the anterior overbite and overjet returned to normal.The canine and molar relationship achieved ClassⅠ,and the profile was improved.The SNA angle decreased by(1.2±1.0)°(P=0.001),and the ANB angle decreased(1.3±0.9)°(P<0.001).The intrusion of maxillary central incisor was(2.4±1.4)mm(U1-PP)(P<0.001),and that of mandibular central incisor was(3.1±1.2)mm(L1-ML)(P<0.001).The sagittal distance which maxillary first molar moved distally was(3.5±0.9)mm(U6-RL)(P<0.001).None of the patients was found with complications such as bone dehiscence,or bone fenestration.The patients having upper and lower incisors with moderate to severe root resorption(>2 mm)were 4.16%after treatment.Conclusions The transmission straight wire orthodontic technique has good effect on non-extraction treatment of adult skeletal ClassⅡmalocclusion.It can decrease the rate of orthodontics complications such as root resorption,bone dehiscence/bone fenestration,etc.
