摘 要:目的评价^(18)F-前列腺特异膜抗原(PSMA)-1007 PET/CT对前列腺癌精囊腺侵犯(SVI)的诊断价值。方法回顾性收集2019年5月至2021年12月88例(51~84岁)在西安交通大学第一附属医院行根治性前列腺癌切除(RP)术的患者的临床和病理资料,所有患者术前行^(18)F-PSMA-1007 PET/CT进行初始分期,以术后病理结果为"金标准",获得^(18)F-PSMA-1007 PET/CT诊断SVI的诊断效能,绘制ROC曲线。采用单因素和多因素logistic回归分析筛选^(18)F-PSMA-1007 PET/CT预测SVI的影响因素。结果^(18)F-PSMA-1007 PET/CT诊断SVI的准确性为79.55%(70/88)、灵敏度为72.73%(16/22)、特异性为81.82%(54/66)、阳性预测值为57.14%(16/28)、阴性预测值为90.00%(54/60),ROC AUC为0.77。单因素logistic回归分析结果显示,总前列腺特异抗原(tPSA)、原发灶SUVmax、Gleason评分、国际泌尿病理协会(ISUP)分级分组与^(18)F-PSMA-1007 PET/CT预测SVI有关;多因素logistic回归分析结果示Gleason评分[比值比(OR)=2.04,95%CI:1.19~3.50,P=0.009]是前列腺癌SVI的预测因素。结论^(18)F-PSMA-1007 PET/CT诊断前列腺癌SVI具有一定价值,结合Gleason评分可以改善诊断效率。Objective To evaluate the diagnostic value of the^(18)F-prostate specific membrane antigen(PSMA)-1007 PET/CT in seminal vesicle invasion(SVI)of prostate cancer.Methods Clinical and pathological materials of 88 patients(age:51-84 years)who underwent radical prostatectomy(RP)between May 2019 and December 2021 in the First Affiliated Hospital of Xi′an Jiaotong University were analyzed retrospectively.All patients underwent^(18)F-PSMA-1007 PET/CT examination for primary staging before surgery.The diagnostic efficiency of^(18)F-PSMA-1007 PET/CT in SVI was obtained using postoperative pathological results as the"gold standard"and ROC curve was drawn.Furthermore,univariate and multivariate logistic regression analyses were used to screen the influencing factors for^(18)F-PSMA-1007 PET/CT prediction of SVI.Results The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of^(18)F-PSMA-1007 PET/CT in diagnosing SVI were 79.55%(70/88),72.73%(16/22),81.82%(54/66),57.14%(16/28)and 90.00%(54/60),respectively.The ROC AUC was 0.77.Results of univariate logistic regression showed that total prostate specific antigen(tPSA),primary SUVmax,Gleason score,International Society of Urological Pathology(ISUP)grade group were associated with^(18)F-PSMA-1007 PET/CT prediction of SVI.Results of multivariate logistic regression showed that Gleason score(odds ratio(OR)=2.04,95%CI:1.19-3.50,P=0.009)was a predictor of SVI in prostate cancer.Conclusion^(18)F-PSMA-1007 PET/CT has certain diagnostic value in SVI of prostate cancer,and combining with Gleason score can improve the diagnostic efficiency.
