摘　　要：目的手辅助腹腔镜手术(HALS)与单纯腹腔镜结直肠癌根治术对结直肠癌(CRC)患者手术指标及康复进程的影响。方法选取2019年2月至2021年2月于本院治疗的60例CRC患者作为研究对象,按照随机数字表法分为HALS组与单纯腹腔镜组,每组30例。HALS组采用手辅助腹腔镜结直肠癌根治术治疗,单纯腹腔镜组采用单纯腹腔镜结直肠癌根治术治疗。比较两组围手术期指标、术后康复时间及并发症发生情况、中转开腹率。结果两组淋巴结清扫数量比较差异无统计学意义;HALS组术中出血量少于单纯腹腔镜组、手术时间短于单纯腹腔镜组,差异有统计学意义(P<0.05)。两组住院时间比较差异无统计学意义;HALS组肠道功能恢复时间、下床时间均短于单纯腹腔镜组,差异有统计学意义(P<0.05)。两组中转开腹率比较差异无统计学意义;HALS组并发症总发生率为6.67%,低于单纯腹腔镜组的26.67%,差异有统计学意义(P<0.05)。结论在CRC患者中采用手辅助腹腔镜结直肠癌根治术手术治疗具有创伤小、康复进程快及并发症少等优点,是治疗CRC的较为理想术式。Objective To investigate the Effects of hand-assisted laparoscopic surgery(HALS)and laparoscopic radical resection of colorectal cancer alone on surgical indicators and rehabilitation process in patients with colorectal cancer(CRC).Methods A total of 60 CRC patients treated in our hospital from February 2019 to February 2021 were selected as the research subjects,and they were divided into the HALS group and the laparoscopy group according to the random number table method,with 30 cases in each group.The HALS group was treated with hand assisted laparoscopic radical resection of colorectal cancer,and the simple laparoscopic group was treated with simple laparoscopic radical resection of colorectal cancer.The perioperative period index,postoperative recovery time and complications were compared between the two groups.Results There was no significant difference in the number of lymph node dissection between the two groups.The intraoperative blood loss in HALS group was less than that in laparoscope group,and the operative time was shorter than that in laparoscope group,and the differences were statistically significant(P<0.05).There was no significant difference in length of stay between the two groups.The recovery time of intestinal function and getting out of bed in HALS group were shorter than those in laparoscopy group,and the difference was statistically significant(P<0.05).There was no significant difference in conversion rate between the two groups.The total incidence of complications in HALS group was 6.67%,which was lower than 26.67%in laparoscopy group,and the difference was statistically significant(P<0.05).Conclusion The use of hand-assisted laparoscopic radical resection of colorectal cancer in CRC patients has the advantages of less trauma,faster recovery and fewer complications,and is an ideal operation for CRC.
