牙髓摘除术与牙髓失活术治疗急性牙髓炎的临床疗效比较

摘　　要：目的:比较牙髓摘除术和牙髓失活术治疗急性牙髓炎的临床疗效、疼痛缓解情况及对其血清炎性因子水平的影响.方法:选取洛阳交通运输集团有限公司职工医院2020年4月至2022年6月收治的96例急性牙髓炎患者作为研究对象.根据不同根管治疗方式将其分为观察组(n=49,接受牙髓摘除术治疗)和对照组(n=47,接受牙髓失活术治疗).比较两组患者临床总有效率、疼痛缓解情况及治疗前后血清炎性因子水平.结果:观察组临床疗效优于对照组,差异有统计学意义(P<0.05).观察组患者治疗过程中及治疗后各时点的疼痛视觉模拟评分(VAS)评分均低于对照组,差异有统计学意义(P<0.05).治疗后,观察组白细胞介素(IL)-6、肿瘤坏死因子(TNF)-α、C反应蛋白(CRP)水平低于对照组,差异有统计学意义(P<0.05).结论:与牙髓失活术相比,牙髓摘除术对急性牙髓炎的治疗效果更好,能有效缓解患牙疼痛和抑制炎性因子的产生,具有临床推广应用价值.Objective:To compare the clinical efficacy,degrees of pain relief,and levels of serum inflammatory factors of pulpectomy and pulp devitalization in the treatment of acute pulpitis.Methods:A total of 96 patients with acute pulpitis admitted to Luoyang Transportation Group Co.,Ltd.Staff Hospital from April 2020 to June 2022 were selected in this study.According to different root canal treatment methods,they were divided into the observation group(n=49,pulpectomy)and the control group(n=47,pulp devitalization).The total clinical efficacy rate,visual analog scale(VAS)scores,and levels of serum interleukin(IL)-6,tumor necrosis factor(TNF)-αand C-reactive protein(CRP)were compared between two groups.Results:The clinical efficacy of observation group was better than that of control group(P<0.05).After treatment,the VAS scores of the observation group at various time points were lower than those of control group(P<0.05).After treatment,the levels of IL-6,TNF-αand CRP in observation group were lower than those in control group(P<0.05).Conclusion:Compared with pulp devitalization,pulpectomy has a better therapeutic efficacy in the treatment of acute pulpitis,which can effectively alleviate patient pain and inhibit the production of inflammatory factors,and is worthy of clinical application.
