醒脑开窍针刺法联合阿替普酶溶栓对急性脑梗死患者炎性反应及血液循环的影响

摘　　要：目的研究醒脑开窍针刺法加阿替普酶溶栓应用于急性脑梗死中的价值。方法100例急性脑梗死患者通过随机数字表法分为两组各50例,研究组使用醒脑开窍针刺法加阿替普酶溶栓,对照组单用阿替普酶溶栓。比较两组总有效率、症状积分、C反应蛋白(CRP)、白细胞介素-8(IL-8)、白细胞介素-10(IL-10)、肿瘤坏死因子-α(TNF-α)、阻力指数(RI)、血流峰值时间(TTP)、平均血流量(Qmean)、脑血容量(CBV)、超氧化物歧化酶(SOD)、丙二醛(MDA)、同型半胱氨酸(Hcy)、血液流变学指标及神经功能缺损(NIHSS)、日常生活能力(ADL)评分。结果研究组总有效率为92.00%,高于对照组的76.00%(P<0.05);研究组治疗后的症状积分、血细胞比容、纤维蛋白原、血小板聚集率、血浆黏度、CRP、IL-8、IL-10、TNF-α、RI、TTP、MDA、Hcy、NIHSS低于对照组(P<0.05),但Qmean、CBV、SOD、ADL高于对照组(P<0.05)。结论醒脑开窍针刺法加阿替普酶溶栓的效果更为理想,能够改善急性脑梗死患者氧化应激、血液循环、血液流变学指标,促进症状快速减轻,缓解炎性反应,促进神经功能及日常生活能力改善。Objective:To study the value of the Xingnao Kaiqiao acupuncture and alteplase thrombolysis in acute cerebral infarction.Methods:A total of 100 patients with acute cerebral infarction were selected and grouped by random number table method,50 cases in each group.The study group received Xingnao Kaiqiao acupuncture and alteplase thrombolysis,and the control group received alteplase thrombolysis.The total response rate,symptom integral,C reactive protein(CRP),interleukin-8(IL-8),interleukin-10(IL-10),tumor necrosis factor-α(TNF-α),resistance index(RI),peak flow time(TTP),mean blood flow(Qmean),cerebral blood volume(CBV),superoxide dismutase(SOD),malondialdehyde(MDA),homocysteine(Hcy),blood rheological index and neurological deficit(NIHSS),and daily living ability(ADL)scores of the two groups were compared.Results:The total response rate of the study group was 92.00%,which was higher than 76.00% of the control group(P<0.05).The symptom score,hematocrit,fibrinogen,platelet aggregation rate,plasma viscosity,CRP,IL-8,IL-10,TNF-α,RI,TTP,MDA,Hcy,NIHSS in the study group were lower than the control group,while Qmean,CBV,SOD,ADL were higher than the control group(P<0.05).Conclusion:The effect of Xingnao Kaiqiao acupuncture and alteplase thrombolysis is more ideal,which can improve oxidative stress,blood circulation and blood rheological indicators of patients,promote the rapid reduction of symptoms,relieve inflammatory reaction,and promote the improvement of neurological function and daily living ability.
