百会八阵穴联合圣愈汤对脑卒中后失语症患者的疗效及脑血流的影响

摘　　要：目的分析百会八阵穴联合圣愈汤对脑卒中后失语症患者的疗效及脑血流的影响。方法回顾性分析2019年1月至2022年1月我院收治的171例脑卒中后失语症患者,根据治疗方式分为百会八阵穴针灸组(对照1组)58例、中药汤剂治疗组(对照2组)70例、联合组43例。对照1组接受百会八阵穴针刺治疗,对照2组采用圣愈汤治疗,联合组采用百会八阵穴针刺联合圣愈汤治疗。比较3组临床疗效、汉语失语症检查表(ABC)评分、失语严重程度、脑血流变化。结果联合组总有效率显著高于对照1组和对照2组(P<0.05);与治疗前相比,3组患者治疗后ABC评分、大脑前动脉血流速度水平、大脑中动脉血流速度水平明显升高(P<0.05),且联合组明显高于对照1组和对照2组(P<0.05);与治疗前相比,3组患者治疗后失语严重程度明显降低(P<0.05),且联合组失语严重程度低于对照1组和对照2组(P<0.05);与治疗前相比,各组患者治疗后大脑后动脉血流速度水平显著升高(P<0.05),但3组大脑后动脉血流速度水平比较无明显差异(P>0.05)。结论百会八阵穴联合圣愈汤治疗脑卒中后失语症患者具有良好的疗效,不但能显著改善患者失语情况,还能有效改善机体脑血流速度。Objective:To analyze the effect of Baihui Bazhen acupoint combined with Shengyu Decoction on aphasia patients after stroke and its influence on cerebral blood flow.Methods:A retrospective analysis was performed on 171 patients with post-stroke aphasia admitted to our hospital from January 2019 to January 2022.According to the treatment methods,they were divided into Baihui Bazhen acupoint acupuncture group(58 cases,control group 1),TCM decoction treatment group(70 cases,control group 2),and combined group(43 cases).Control group 1 was treated with Baihui Bazhen acupoint acupuncture,control group 2 was treated with Shengyu Decoction,and combination group was treated with Baihui Bazhen acupoint acupuncture combined with Shengyu Decoction.The clinical efficacy,ABC score,aphasia severity and cerebral blood flow of the three groups were compared.Results:The clinical efficacy of combination group was significantly higher than those of control group 1 and control group 2(P<0.05).Compared with before treatment,ABC score,anterior cerebral artery(ACA)level and middle cerebral artery(MCA)level in three groups were significantly increased after treatment(P<0.05),and the combination group was significantly higher than control group 1 and control group 2(P<0.05).Compared with before treatment,the severity of aphasia in the three groups was significantly decreased after treatment(P<0.05),and the severity of aphasia in the combination group was lower than that in control group 1 and control group 2(P<0.05).Compared with before treatment,PCA level was significantly increased after treatment(P<0.05),but there was no significant difference in posterior cerebral artery level among the three groups(P>0.05).Conclusion:Baihui Bazhen acupoint combined with Shengyu Decoction has a good effect in the treatment of aphasia patients after stroke,which can not only significantly improve the aphasia condition of patients,but also effectively improve the speed of cerebral blood flow.
