摘 要:目的:探讨单光子发射计算机断层成像术(single-photon emission computed tomography,SPECT)/电子计算机X射线断层扫描技术(computed tomography,CT)融合显像在肺癌骨转移患者中的诊断价值。方法:选取新余市人民医院核医学科2022年2月—2023年1月接收的80例疑似肺癌骨转移患者,入选患者均进行CT、磁共振成像(MRI)及SPECT/CT融合显像检查,最终结果与手术病理金标准相比,计算三种检查方法诊断效能(敏感度、特异度、准确率),三者对不同肺癌原发病灶的检出率。结果:80例肺癌骨转移患者中,经手术病理确诊病灶为93个,包括恶性病变72个(<1 cm的15个,1~3 cm的33个,>3 cm的24个),良性病变21个;CT诊断肺癌骨转移的敏感度、特异度、准确率分别为69.44%、76.19%、70.97%;MRI分别为80.56%、71.43%、78.49%;SPECT/CT融合显像分别为97.22%、85.71%、94.62%,但其中以SPECT/CT融合显像检查敏感度、准确率显著更高(P<0.05);SPECT/CT对直径<1 cm肺癌原发病灶检出率高于CT、MRI,差异有统计学意义(P<0.05)。结论:临床进行肺癌骨转移诊断可采取MRI、CT及SPECT/CT融合显像,但SPECT/CT融合现象相较于CT、MRI诊断而言,诊断价值更高,且在微小病灶的检出上更具优势。Objective:To investigate the diagnostic value of single-photon emission computed tomography(SPECT)/computed tomography(CT)fusion imaging in patients with bone metastases of lung cancer.Method:A total of 80 patients who were suspected of bone metastases of lung cancer and admitted to the Nuclear Medicine Department of Xinyu People's Hospital from February 2022 to January 2023 were selected.All patients were examined with CT,magnetic resonance imaging(MRI)and SPECT/CT fusion imaging.With surgical and pathological results as the golden standard,the diagnostic performance(sensitivity,specificity and accuracy)of the three examination methods were evaluated.The detection rates for different primary lung cancer lesions were calculated.Result:Among the 80 patients with bone metastases of lung cancer,93 lesions were confirmed by surgery and pathology,including 72 malignant lesions(15<1 cm,33 of 1-3 cm,24>3 cm)and 21 benign lesions.The sensitivity,specificity and accuracy of CT for diagnosing bone metastases of lung cancer were 69.44%,76.19%and 70.97%,the MRI were 80.56%,71.43%and 78.49%,the SPECT/CT fusion imaging were 97.22%,85.71%and 94.62%.The sensitivity and accuracy of SPECT/CT fusion imaging were significantly higher(P<0.05).The detection rate of primary lung cancer lesions<1 cm using SPECT/CT was higher than that using CT or MRI,with statistically significant differences(P<0.05).Conclusion:MRI,CT,and SPECT/CT fusion imaging can be used for diagnosing bone metastases of lung cancer in clinical practice.Compared with CT or MRI,SPECT/CT fusion imaging has more advantages in detecting small lesions.
