摘 要:目的 探讨外周血补体成分C3c、C4、C1q水平与年龄相关性白内障(ARC)发生、发展的相关性。方法 采用病例对照研究设计,收集2016—2020年本院确诊为ARC的患者和年龄、性别相匹配的对照组人群的基础资料。采用罗氏全自动生化仪检测ARC组及对照组血清C3c、C4、C1q。ARC组与对照组间补体指标比较采用独立样本t检验,Spearman相关性分析用于分析各指标与ARC之间的相关性,二元Logistic回归用于分析与ARC相关的危险因素。结果 共纳入ARC患者2 321例(男/女:1 115/1 206);对照组共纳入2 281例(男/女:1 112/1 169)。独立样本t检验显示:ARC组血清C1q为(219.27±65.03)mg/L,显著高于对照组的(207.87±48.49)mg/L;C3c、C4分别为(104.13±17.28)mg/dL、(26.85±7.99)mg/dL,显著低于对照组的(112.81±17.96)mg/dL、(28.08±8.37)mg/dL,差异有统计学意义(P<0.05)。相关性分析结果显示:血清C1q水平与ARC存在显著正相关性(r=0.062,P<0.001);血清C3c(r=-0.243,P<0.001)、C4(r=-0.081,P<0.001)水平与ARC存在显著负相关性;回归分析显示,外周血C1q水平(男性:OR=1.007,95%CI为1.005~1.008;女性:OR=1.001,95%CI为1.000~1.003)升高是ARC发生的危险因素。结论 ARC患者外周血补体C3c、C4、C1q水平发生显著变化,是ARC发病的危险因素,提示补体系统可能参与ARC的病变。Objective To explore the correlation between the levels of complement(C3c,C4,C1q)in peripheral blood and the occurrence and development of cataract in patients with age-related cataracts(ARC).Methods This is a case-control study.Subjects diagnosed with ARC between 2016 and 2020 were recruited from Eye&ENT Hospital of Fudan University.Age-and gender-matched control group was recruited during the same period.The basic information were collected.Serum levels of C3c,C4,C1q were measured by Roche automatic biochemical analyzer.The independent student’s t-test was used for the comparison of C3,C4 and C1q between the patients and the control group.The associations between C3c,C4,C1q and ARC were analyzed by Spearman correlation and binary logistic regression.Results A total of 2321 ARC patients(male 1115,female 1206)and 2281 controls(male 1112,female 1169)were enrolled in this study.Independent sample t-test showed that the serum levels of C3c,C4(104.13±17.28 mg/dL,26.85±7.99 mg/dL)were significantly lower in the ARC group compared with the control group(112.81±17.96 mg/dL,28.08±8.37 mg/dL);the serum level of C1q(219.27±65.03 mg/L)were significantly higher in the ARC group compared with the control group(207.87±48.49 mg/L)(P<0.05).Correlation analysis showed that the level of serum C1q was postively correlated with ARC(r=0.062,P<0.001),while the levels of serum C3c(r=-0.243,P<0.001),C4(r=-0.081,P<0.001)were negatively correlated with ARC(P<0.05).Logistic regression analysis showed that the increased serum level of C1q(male:OR=1.007,95%CI=1.005~1.008;female:OR=1.001,95%CI=1.000~1.003)was the risk factor for the occurrence of ARC(P<0.05).Conclusions The levels of C3c,C4 and C1q in peripheral blood of ARC subjects were significantly changed and the increased serum level of C1q may be the risk factor for ARC.It indicated that complement system might be involved in the pathogenesis of ARC.
