摘　　要：目的 探讨结直肠癌(CRC)患者术前血脂水平与术后病理分期的相关性。方法 回顾性分析2019-2020年该院收治并经手术病理诊断明确的360例CRC患者的临床资料,包括姓名、性别、年龄、术前血脂[总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)、脂蛋白α、载脂蛋白AⅠ、载脂蛋白B]水平、术后病理分期、肿瘤位置等,分析CRC患者术前血脂水平对术后病理分期的影响。结果 血脂水平正常与否在性别分布上有差异(P<0.05),在年龄及肿瘤位置分布上差异无统计学意义(P>0.05)。HDL及载脂蛋白AⅠ水平在T分期、M分期、TNM分期的分布上存在统计学差异(P<0.05)。logistic回归分析显示:LDL水平是病理分期的危险因素(P<0.05),TC、HDL、载脂蛋白AⅠ水平是病理分期的保护因素(P<0.05)。结论 血脂水平与CRC的发生、发展密切相关。Objective To investigate the correlation between preoperative plasma lipid level and postoperative pathological stage in patients with colorectal cancer(CRC).Methods A retrospective analysis was performed on the clinical data of 360 patients with colorectal cancer admitted to this hospital from 2019 to 2020 and confirmed by surgical and pathological diagnosis,including name,sex,age,preoperative plasma lipid level[total cholesterol(TC),triglycerides(TG),low density lipoprotein(LDL),high density lipoprotein(HDL),Lipoproteinα,apolipoprotein AⅠ,apolipoprotein B],postoperative pathological stage,tumor location,etc.The influence of preoperative plasma lipid level on postoperative pathological stage was statistically analyzed.Results Whether the plasma lipid level was normal or not was different in gender distribution(P<0.05),but there was no significant difference in age and tumor location distribution(P>0.05).The levels of HDL and apolipoprotein AI were significantly different in T stage,M stage and TNM stage(P<0.05).Logistic regression analysis showed that the levels of TC,LDL,HDL and apolipoprotein AI were closely related to the pathological stage of colorectal cancer;The level of LDL was the risk factor of pathological stage,while the levels of TC,HDL and apolipoprotein AI were the protective factors of pathological stage.Conclusion Plasma lipid level is closely related to the occurrence and development of CRC.
