摘 要:目的 评价加涅教学理论对于颈内静脉穿刺置管术教学的影响。方法 选择2021年9月—2022年6月本院的临床实习学生70名作为教学对象,随机分配为实验组和对照组两组,每组各35名。实验组由课题组教师带教,按照加涅教学理论进行教学。教学前将颈内静脉穿刺置管术进行任务分析,归纳出该操作的执行子程序并总结出该操作所要求的分技能。然后按项教授学生分技能,最后学生按照操作步骤和顺序独立进行完整操作。对照组按照传统讲授模式教学。结果 教学后考核及问卷调查,实验组的理论知识(81.0±7.3)分、技能操作(89.2±6.1)分及总成绩(170.2±8.0)分,均高于对照组的(77.1±8.2)分、(74.8±9.1)分以及(151.9±10.7)分。问卷调查实验组与对照组在调动学习积极性(100.0%VS 77.1%)、提高自学能力(88.6%VS 65.7%)、提高技能操作能力(100.0%VS 62.9%)、自我信心提升(94.3%VS 71.4%)、提高交流表达能力(82.9%VS 60.0%)、综合满意度(88.6%VS 62.9%)方面均有统计学差异,实验组高于对照组(P<0.05)。结论 加涅的教学理论和教学设计理念,对颈内静脉穿刺置管术操作教学的帮助很大。Objective To evaluate the influence of Gagne's teaching theory on the teaching of internal jugular vein puncture and catheterization.Methods 70 students participating clinical practice in our hospital from September 2021 to June 2022 were selected as the teaching objects,and they were randomly divided into the experimental group and the control group,with 35 students in each group.The experimental group was taught by the teachers of the research group according to Gagne's teaching theory.Before teaching,the task analysis of internal jugular vein puncture and catheterization was carried out,the execution subprogram of the operation was summarized and the required sub-skills of the operation were summarized.Then the students were taught the skills according to the items,and finally the students completed the operation independently according to the operation steps and order.The control group was taught according to the traditional teaching mode.Results The scores of theoretical knowledge(81.0±7.3),skill operation(89.2±6.1)and total score(170.2±8.0)in the experimental group were higher than those in the control group(77.1±8.2),(74.8±9.1)and(151.9±10.7).In the questionnaire survey,there were statistically significant difference between experimental group and the control group in mobilizing learning enthusiasm(100.0%VS 77.1%),improving self-learning ability(88.6%VS 65.7%),improving skill operation ability(100.0%VS 62.9%),and improving self-confidence(94.3%VS 94.3%)71.4%),communication skills(82.9%VS 60.0%)and comprehensive satisfaction(88.6%VS 62.9%),those in the experimental group were higher than those in the control group.Conclusions Gagane's teaching theory and teaching design concept are very helpful to the teaching of internal jugular vein puncture and catheterization.
