个案管理在膀胱癌输尿管皮肤造口患者中的应用效果

摘　　要：目的:探析个案管理在膀胱癌输尿管皮肤造口患者中的应用效果。方法:选取2022年12月—2023年1月山东第一医科大学附属山东省肿瘤医院收治的行膀胱癌输尿管皮肤造口术患者70例为观察对象,采用随机数字表法分为观察组、对照组,各35例。对照组实施常规管理,观察组实施个案管理。比较两组干预前后患者自我护理能力、膀胱癌患者生活质量特异模块(QLQ-BLM30)评分、日常生活能力(Barthel指数)。结果:干预后,观察组情感接受程度、操作技能评分高于对照组,差异有统计学意义(P<0.05);干预后,两组QLQ-BLM30评分均降低,且观察组低于对照组,差异有统计学意义(P<0.05);干预后,两组Barthel评分均升高,且观察组高于对照组,差异有统计学意义(P<0.05)。结论:个案管理应用于膀胱癌输尿管皮肤造口患者中临床效果显著,可有效提高患者自我护理能力、生活质量及日常生活能力。Objective:To explore the application effect of case management in ureteral dermatostomy of bladder cancer.Methods:Seventy patients undergoing ureteral dermatostomy for bladder cancer treated in Shandong Cancer Hospital Affiliated to Shandong First Medical University from December 2022 to January 2023 were selected as the observation objects,and were divided into ob⁃servation group and control group by random number table method,with thirty-five cases in each group.The control group imple⁃mented routine management,and the observation group implemented case management.Self-care ability,quality-of-life for Blad⁃der Cancer Module(QLQ-BLM30)score and daily living ability(Barthel index)were compared between the two groups before and after intervention.Results:After the intervention,the scores of emotional acceptance and operation skill in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05);After intervention,the QLQBLM30 score of both groups was decreased,and the observation group was lower than the control group,the difference was statisti⁃cally significant(P<0.05);After intervention,the Barthel score of both groups was increased,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).Conclusion:The application of case management in pa⁃tients with ureteral dermatostomy for bladder cancer has significant clinical effect,which can effectively improve patients'selfcare ability,quality of life and daily living ability.
