双歧杆菌四联活菌片辅助标准四联疗法治疗Hp阳性胃溃疡的效果及对患者胃肠功能、免疫功能及炎症反应的影响

摘　　要：目的分析双歧杆菌四联活菌片辅助标准四联疗法治疗幽门螺旋杆菌(Hp)阳性胃溃疡的效果。方法将2019年12月至2021年12月收治的84例Hp阳性胃溃疡患者随机分为对照组(42例,奥美拉唑+克拉霉素+阿莫西林+胶体果胶铋的标准四联疗法)和观察组(42例,在对照组基础上给予双歧杆菌四联活菌片治疗)。比较两组的治疗效果。结果观察组的治疗总有效率、Hp转阴率、溃疡愈合率均高于对照组(P<0.05)。治疗后,观察组的胃动素(MTL)、胃泌素(GAS)、胃蛋白酶原Ⅰ(PGⅠ)、血清淀粉样蛋白A(SAA)、超敏C反应蛋白(hs-CRP)、基质金属蛋白酶-9(MMP-9)水平低于对照组,生长抑素(SS)水平高于对照组(P<0.05)。治疗后,观察组的白细胞分化抗原3阳性(CD3^(+))、白细胞分化抗原4阳性(CD4^(+))高于对照组,调节性T细胞(Treg)、调节性B细胞(Breg)低于对照组(P<0.05)。结论双歧杆菌四联活菌片辅助标准四联疗法治疗Hp阳性胃溃疡患者具有更好疗效,可提高Hp转阴率和溃疡愈合率,改善胃肠功能指标水平,提高免疫功能,减轻炎症反应。Objective To analyze the effect of combined Bifidobacterium,Lactobacillus,Enterococcus and Bacillus cereus tablets,live assisted standard quadruple therapy in the treatment of Helicobacter pylori(Hp)positive gastric ulcer.Methods A total of 84 patients with Hp positive gastric ulcer admitted from December 2019 to December 2021 were randomly divided into control group(42 cases,standard quadruple therapy of omeprazole+clarithromycin+amoxicillin+colloidal bismuth pectin)and observation group(42 cases,combined Bifidobacterium,Lactobacillus,Enterococcus and Bacillus cereus tablets,live on the basis of the control group).The therapeutic effects of the two groups were compared.Results The total effective rate of treatment,Hp negative conversion rate and ulcer healing rate of the observation group were higher than those of the control group(P<0.05).After treatment,the levels of motilin(MTL),gastrin(GAS),pepsinogenⅠ(PGⅠ),serum amyloid A(SAA),hypersensitive C-reactive protein(hs-CRP)and matrix metalloproteinase-9(MMP-9)in the observation group were lower than those in the control group,and the level of somatostatin(SS)was higher than that in the control group(P<0.05).After treatment,the cluster of differentiation 3 positive(CD3^(+))and cluster of differentiation 4 positive(CD4^(+))in the observation group were higher than those in the control group,and the regulatory T cells(Treg)and regulatory B cells(Breg)were lower than those in the control group(P<0.05).Conclusion Combined Bifidobacterium,Lactobacillus,Enterococcus and Bacillus cereus tablets,live assisted standard quadruple therapy has better efficacy in the treatment of Hp positive gastric ulcer of patients,which can improve Hp negative conversion rate and ulcer healing rate,improve gastrointestinal function indexes levels and immune function,and reduce inflammatory response.
