摘 要:目的研究采用低速直机联合高速涡轮机微创拔牙治疗复杂阻生牙的临床价值。方法80例复杂阻生牙患者,采取随机数字表法分为对照组和观察组,每组40例。对照组行传统骨凿法拔牙治疗,观察组行低速直机联合高速涡轮机微创拔牙治疗。比较两组患者手术情况[手术时间、视觉模拟评分法(VAS)评分、张口受限程度、创面肿胀程度、牙槽窝完整性评分]以及术后并发症发生情况。结果观察组患者术后并发症发生率为5.00%,低于对照组的20.00%,差异具有统计学意义(P<0.05)。观察组手术时间(14.50±1.80)min短于对照组的(21.80±2.50)min,VAS评分(1.30±0.50)分、张口受限程度(3.50±0.50)mm、创面肿胀程度(14.10±0.50)mm、牙槽窝完整性评分(1.30±0.20)分均低于对照组的(2.50±0.60)分、(8.80±1.20)mm、(18.90±1.20)mm、(3.50±0.50)分,差异具有统计学意义(P<0.05)。结论低速直机联合高速涡轮机微创拔牙治疗复杂阻生牙效果显著,有助于减少患者术后牙龈撕裂、感染等并发症发生情况,患者恢复效果好。Objective To study the clinical value of low-speed straight turbine combined with highspeed turbine for minimally invasive tooth extraction in the treatment of complex impacted teeth.Methods A total of 80 patients with complex impacted teeth were divided into control group and observation group according to the random numerical table,with 40 cases in each group.The control group underwent traditional osteotome tooth extraction,and the observation group underwent low-speed straight turbine combined with high-speed turbine for minimally invasive tooth extraction.Both groups were compared in terms of surgical status[operative time,visual analogue scale(VAS)score,restricted mouth opening,wound swelling,alveolar socket integrity score],and postoperative complications.Results The incidence of postoperative complications in the observation group was 5.00%,which was lower than that of 20.00%in the control group,and the difference was statistically significant(P<0.05).The operative time of(14.50±1.80)min in the observation group was shorter than that of(21.80±2.50)min in the control group;in the observation group,VAS score was(1.30±0.50)points,the restricted mouth opening was(3.50±0.50)mm,the wound swelling was(14.10±0.50)mm,and the alveolar socket integrity score was(1.30±0.20)points,which were lower than those of(2.50±0.60)points,(8.80±1.20)mm,(18.90±1.20)mm,and(3.50±0.50)min in the control group;the differences were statistically significant(P<0.05).Conclusion Low-speed straight turbine combined with high-speed turbine minimally invasive tooth extraction has significant effect for the treatment of complex impacted teeth,which helps to reduce the postoperative complications such as gum tear and infection,and the patient recovers well.
