摘　　要：目的探讨单纯疱疹病毒2型(HSV-2)感染与急性缺血性脑卒中(AIS)患者对配体半乳凝素-9(Galectin 9)/T细胞免疫球蛋白黏蛋白分子-3(Tim-3)信号通路及预后的关系。方法回顾性收集2018年1月-2021年1月南阳医学高等专科学校第一附属医院75例AIS患者临床资料,根据HSV-2免疫球蛋白(Ig)G水平,按四分位数法将AIS患者分为Ⅰ、Ⅱ、Ⅲ和Ⅳ组,分别为21、18、19和17例,以Ⅰ组为对照组。对比四组临床资料、Galectin 9/Tim-3信号通路分子表达量以及预后结局,同时通过Logistic回归分析明确HSV-2感染与AIS患者预后相关性。结果入院美国国立卫生研究院卒中量表评分(NIHSS):Ⅳ组>Ⅲ组、Ⅱ组>Ⅰ组(P<0.05);C-反应蛋白(CRP):Ⅳ组>Ⅲ组、Ⅱ组>Ⅰ组(P<0.05);白细胞计数(WBC):Ⅳ组>Ⅲ组、Ⅱ组、Ⅰ组(P<0.05)。Galectin 9 mRNA、CD4+Tim-3表达水平:Ⅳ组>Ⅲ组>Ⅱ组>Ⅰ组(P<0.05);CD8+Tim-3表达水平:Ⅳ组>Ⅲ组>Ⅱ组、Ⅰ组;Tim-3 mRNA表达水平:Ⅳ组>Ⅲ组、Ⅱ组>Ⅰ组(P<0.05)。Ⅳ组患者预后不良结局总发生率高于Ⅰ组、Ⅱ组、Ⅲ组(P<0.05)。Logistic回归分析显示,HSV感染与AIS患者不良预后相关(P<0.05)。结论AIS患者HSV-2感染可一定程度提高其Galectin 9/Tim-3信号通路分子表达量,同时HSV-2感染严重程度是AIS患者预后不良结局发生风险增加的关键因素。OBJECTIVE To explore the relationship between herpes simplex virus type 2(HSV-2)infection and Galectin-9/T-cell immunoglobulin mucin-3(Tim-3)signaling pathways,prognosis of the patients with acute ischemic stroke(AIS).METHODS The clinical data were retrospectively collected from 75patients with AIS who were treated in the First Affiliated Hospital of Nanyang Medical College from Jan 2018to Jan 2021.The AIS patients were divided into the groupⅠwith 21cases,the groupⅡwith 18cases,the groupⅢwith 19cases,and the groupⅣwith 17cases according to the HSV-2immunoglobin(Ig)G level.The groupⅠwas set as the control group.The clinical data,expression levels of Galectin 9/Tim-3signaling pathways and prognosis were compared among the four groups.Logistic regression analysis was performed for the relationship between the HSV-2infection and the prognosis of the AIS patients.RESULTS The score of National Institutes of Health Stroke Scale(NIHSS)of theⅣgroup was higher than that of the groupⅢ,the score of NIHSS of the groupⅡwas higher than that of the groupⅠ(P<0.05).The C-reactive protein(CRP)level of the groupⅣwas higher than that of the groupⅢ,the CRP level of the groupⅡwas higher than that of the groupⅠ(P<0.05).The white blood cell(WBC)level of the groupⅣwas higher than that of the groupⅢ,the groupⅡand the groupⅠ(P<0.05).The expression levels of Galectin 9mRNA and CD4+Tim-3of the four groups were as follows:the groupⅣ>the groupⅢ>the groupⅡ>the groupⅠ(P<0.05);the expression level of CD8+Tim-3of the four groups was as follows:the groupⅣ>the groupⅢ>the groupⅡ,the groupⅠ;the expression level of Tim-3mRNA of the groupⅣwas higher than that of the groupⅢ,and the expression level of Tim-3mRNA of the groupⅡwas higher than that of the groupⅠ(P<0.05).The total incidence of adverse treatment outcomes of the groupⅣwas higher than that of the groupⅠ,ⅡandⅢ(P<0.05).Logistic regression analysis showed that the HSV infection was associated with the adverse prognosis of the AIS pati
