摘  要:目的探讨甘肃省部分地区食管癌发病危险因素,为甘肃省食管癌的预防提供依据。方法选取2019年至2022年就诊于甘肃省肿瘤医院、甘肃省人民医院、兰州大学第一医院经胃镜、病理检查确诊的新发食管癌患者311例作为观察组,采用1∶1配对方法于非食管癌人群中选取对照组311例。采用Logistic回归分析对食管癌发病危险因素进行研究。结果经单因素分析发现口腔疾病、长期吸烟、长期饮酒、饮茶、进食速度快、暴饮暴食、重盐饮食、常食热烫食物、常食硬质食物、常食腌菜、常食油炸烟熏食物、常食霉变食物、饮用非自来水与当地食管癌发病相关(P<0.05),经济收入、水果、蔬菜、豆制品是食管癌发病的保护因素。对单因素有意义的因子进一步多因素分析得出:口腔疾病、吸烟、饮酒、饮茶、常食热烫食物、常食腌菜、常食油炸烟熏食物、进食速度快、暴饮暴食、饮用非自来水是食管癌发病的危险因素。结论甘肃省食管癌发病与吸烟、饮酒、不良生活和饮食习惯密切相关。Objective To investigate the risk factors of esophageal cancer in specific regions of Gansu province and provide evidence for its prevention.Methods A total of 311 newly diagnosed esophageal cancer patients admitted to Gansu Cancer Hospital,Gansu Provincial People’s Hospital,and the First Hospital of Lanzhou University from 2019 to 2022 were selected as the case group.A control group of 311 patients without esophageal cancer was selected using a 1∶1 matching method.Logistic regression analysis was conducted to analyze the risk factors for esophageal cancer.Results Single-factor analysis revealed that oral diseases,long-term smoking,long-term drinking,tea consumption,fast eating,overeating,a diet high in salt,consumption of hot and hard foods,pickles,fried and smoked foods,consumption of moldy food,and drinking non-tap water were significantly associated with the incidence of esophageal cancer in the local population(P<0.05).Income,fruits,vegetables,and soy products were found to be protective factors against esophageal cancer.Further multivariate analysis of the factors that showed significance in the single-factor analysis revealed that oral diseases,smoking,drinking,tea consumption,frequent consumption of hot food,regular consumption of pickled vegetables,frequent consumption of oil-fried and smoked food,fast eating,overeating,and drinking non-tap water are risk factors for esophageal cancer.Conclusion The incidence of esophageal cancer in Gansu province is closely associated with smoking,drinking,unhealthy living habits,and poor dietary choices.
