摘　　要：目的分析脑出血患者住院费用构成情况及影响因素,为有效控制脑出血患者费用增长,降低经济负担提供参考。方法收集西安市某三甲医院2015年1月1日-2020年12月31日主要诊断为脑出血患者的住院病历信息,对其进行统计描述,通过单因素分析、多元线性回归分析探索住院费用的影响因素。结果脑出血患者的人均住院费用为39277.70元,是所有病种人均住院费用的3倍,住院费用的中位数为21975.04元。住院费用构成中占比最高的为药费(34.7%),其次为检查费(20.7%)和材料费(17.9%)。多元线性回归分析显示住院费用的主要影响因素依次为是否手术、住院天数、合并症个数和年龄,P<0.001。结论脑出血患者的住院费用逐年增长,手术和住院天数是影响住院费用的主要因素。提高脑出血的防治意识,缩短住院天数,批量采购耗材等,可有效控制住院费用增长。Objectives This study aims to analyze the composition and influencing factors of hospitalization expenses of patients with cerebral hemorrhage,so as to provide a reference for effectively controlling the expenses growth of patients with cerebral hemorrhage and reducing the economic burden.Methods The medical records of patients mainly diagnosed as cerebral hemorrhage in a tertiary hospital in Xi'an from January 1st,2015 to December 31st,2020 were collected,and the statistical description was carried out.The influencing factors of hospitalization expenses were investigated through univariate analysis and multiple linear regression analysis.Results The per capita hospitalization expense of patients with cerebral hemorrhage was 39277.70 yuan,which was three times that of other diseases.The median hospitalization expense was 21975.04 yuan.The highest proportion of hospitalization expenses was drug cost(34.7%),followed by examination cost(20.7%)and material cost(17.9%).Multiple linear regression analysis showed that the main influencing factors of hospitalization expense were surgery,hospitalization days,number of comorbidities,and age(P<0.001).Conclusions The hospitalization expense of patients with cerebral hemorrhage is increasing year by year.Surgery and length of hospital stay are the main factors affecting hospital costs.The growth of hospitalization expenses can be effectively controlled by improving the awareness of prevention and treatment of cerebral hemorrhage,shortening hospitalization days,purchasing consumables in batches,etc.
