血清DCLK1、sTim-3对前列腺癌根治术患者预后的评估价值

摘　　要：目的 探讨血清双皮质素样激酶1(DCLK1)、可溶性T细胞免疫球蛋白黏蛋白分子3(sTim-3)对前列腺癌根治术患者预后的评估。方法 选取2016年6月至2019年6月在右江民族医学附属医院行前列腺癌根治术的80例患者作为研究组,另选取同期80例健康体检者作为对照组;采用酶联免疫吸附(ELISA)法检测血清DCLK1、sTim-3水平;分析血清DCLK1、sTim-3与前列腺癌根治术患者病理临床特征及预后的关系。结果 研究组血清DCLK1、sTim-3水平显著高于对照组(P <0.05)。血清DCLK1、sTim-3水平呈正相关(P <0.05)。二者与PSA水平、Gleason评分均呈正相关(P <0.05)。血清DCLK1、sTim-3表达水平与PSA水平、Gleason评分以及TNM分期有关(P <0.05)。预后死亡组血清DCLK1、sTim-3水平显著高于预后生存组(P <0.05)。DCLK1和sTim-3高表达患者生存率低于低表达患者。根据Cox回归分析表明,DCLK1、sTim-3高表达是影响前列腺癌根治术患者预后的危险因素(P <0.05)。根据ROC曲线得知,二者联合预测前列腺癌根治术患者预后的AUC优于DCLK1、sTim-3各自单独预测。结论 DCLK1、sTim-3在前列腺癌患者血清中显著升高,二者联合可以更好对前列腺癌根治术患者预后进行评估。Objective To investigate the prognosis of serum bicorticotin-like kinase 1(DCLK1)and soluble T cell immunoglobulin mucin molecule 3(sTim-3)in patients undergoing radical prostatectomy.Methods This study selected 80 patients who underwent radical prostatectomy in our hospital from June 2016 to June 2019 as the research group,and another 80 healthy individuals who underwent physical examination during the same period as the control group;enzyme linked immunosorbent assay(ELISA)was applied to detect serum levels of DCLK1 and sTim-3;The relationship between serum DCLK1 and sTim-3 and pathologic features and prognosis of patients undergoing radical prostatectomy was analyzed.Results The serum levels of DCLK1 and sTim-3 in the study group were obviously higher than those in the control group(P<0.05).Serum DCLK1 and sTim-3 levels were positively correlated(P<0.05).Both were positively correlated with PSA level and Gleason score(P<0.05).Serum DCLK1 and sTim-3 expression levels were associated with PSA levels,Gleason scores,and TNM stages(P<0.05).The serum levels of DCLK1 and sTim-3 in the prognostic death group were significantly higher than those in the prognostic survival group(P<0.05).Patients with high expression of DCLK1 and sTim-3 had lower survival rates than those with low expression.According to Cox regression analysis,high expression of DCLK1 and sTim-3 was a risk factor affecting the prognosis of patients undergoing radical prostatectomy(P<0.05).According to the ROC curve,the AUC that jointly predicted the prognosis of patients undergoing radical prostatectomy was better than that of DCLK1 and sTim-3 respectively.Conclusion DCLK1 and sTim-3 are obviously elevated in the serum of prostate cancer patients,and their combination can better evaluate the prognosis of patients undergoing radical prostatectomy.
