摘　　要：目的 探讨动态对比增强MRI分型对乳腺癌患者预后的预测价值。方法 对84例乳腺癌患者进行动态对比增强MRI检查并进行分型,随访患者的预后并进行相关性、预测价值分析。结果 两组患者动态对比增强MRI分型比较,差异有统计学意义(Z=6.008,P﹤0.01);复发组患者动态对比增强MRI分型多为Ⅲ型与Ⅳ型,非复发组多为Ⅱ型与Ⅲ型。Spearman相关分析结果显示,动态对比增强MRI分型中Ⅲ型+Ⅳ型与预后复发呈正相关(r=0.413,P﹤0.01)。受试者工作特征(ROC)曲线分析显示,动态对比增强MRI分型预测乳腺癌患者预后复发的曲线下面积为0.763(95%CI:0.651~0.875)。结论 乳腺癌的复发率一直居高不下,动态对比增强MRI分型可有效预测患者的预后复发情况,也与患者的预后复发存在相关性。Objective To explore the predicting value of dynamic contrast-enhanced MRI classification in the progno-sis of breast cancer.Method A total of 84 patients with breast cancer were given dynamic contrast-enhanced MRI exami-nation and classified.The patients were followed up for prognosis and correlation and predictive value were analyzed.Re-sult There was significant difference for the classification by dynamic contrast-enhanced MRI in the recurrence group and non-recurrence group(Z=6.008,P<0.01).The dynamic contrast enhanced MRI types of patients in the recurrence group were mostly type III and type IV,while those in the non-recurrence group were mostly type II and type III.Spear-man correlation analysis showed that type III+type IV in dynamic contrast-enhanced MRI classification was positively correlated with prognostic recurrence(r=0.413,P<0.01).Receiver operating characteristic(ROC)curve analysis showed that the area under the curve of dynamic contrast enhanced MRI classification for predicting prognostic recurrence in breast cancer patients was 0.763(95%CI:0.651-0.875).Conclusion The recurrence rate of breast cancer remains high.Dynamic contrast-enhanced MRI classification can effectively predict the prognostic recurrence of patients,which is also correlated with the prognostic recurrence of patients.
