保乳术与改良根治术治疗早期乳腺癌的临床效果及对患者生活质量的影响

摘　　要：目的 研究对比早期乳腺癌行保乳术、改良根治术对临床疗效和生活质量的影响。方法 回顾性分析2021年1月—2022年1月河南省人民医院乳腺外科收治的100例早期乳腺癌患者的临床资料,将行保乳术治疗的48例患者设为研究组,而行改良根治术治疗的52例患者设为对照组。对比两组手术指标、乳房美观度、术后并发症、肿瘤标志物水平、生活质量和远期疗效。结果 对照组手术指标与研究组比差异有统计学意义(P<0.05);研究组乳房美观度显著高于对照组,存在明显差异(P<0.05);对照组术后并发症(17.31%)与研究组(4.17%)比较有统计学价值(P<0.05);和对照组比较研究组肿瘤标志物水平更低,但差异性不显著(P>0.05);对照组生活质量综合评定问卷(GQOLI-74)评分低于研究组,存在显著差异性(P<0.05);研究组远期疗效和对照组比较,差异无统计学价值(P>0.05)。结论 对早期乳腺癌患者实施保乳术能优化手术指标,保障乳房美观度,还能减少并发症少,使患者生活质量得到提高,整体疗效优于改良根治术,值得推荐。Objective To study and compare the effects of breast conserving surgery and modified radical surgery on clinical efficacy and quality of life of patients with early breast cancer.Methods The clinical data of 100 patients with early breast cancer from the Department of Breast Surgery from January 2021 to January 2022 were retrospectively ana-lyzed.48 patients receiving breast-conserving surgery were set as the study group,and 52 patients receiving modified radical surgery were set as the control group.Surgical indexes,breast aesthetics,postoperative complications,tumor markers,quality of life and long-term efficacy were compared between the two groups.Results There was significant difference between the control group and the study group(P<0.05).The breast beauty of the study group was signifi-cantly higher than that of the control group(P<0.05).The postoperative complications of the control group were signif-icantly higher than those of the study group(P<0.05).Compared with the control group,the level of tumor markers in the study group was lower,but the dfference was not significant(P>0.05).The score of GQOLI-74 in control group was significantly lower than that in study group(P<0.05).There was no significant difference in long-term efficacy between the study group and the control group(P>0.05).Conclusion The implementation of breast conserving sur-gery for early breast cancer patients can optimize the surgical indicators,ensure the beauty of the breast,reduce compli-cations,improve the quality of life of patients,the overall efficacy is better than the modified radical surgery,worthy of recommendation.
