食管癌手术中应用细管状胃食管吻合治疗对患者肺功能及生活质量的影响

摘　　要：目的 分析细管状胃食管吻合治疗对食管癌患者肺功能、生活质量以及并发症的影响。方法 回顾性病例对照研究,选取2020年1月-2022年9月郑州大学第一附属医院收治的食管癌患者76例,按手术方法不同,将采用粗管状胃代食管吻合术治疗的38例作为对照组,将采用细管状胃代食管吻合术治疗的38例作为研究组,对比两组患者治疗情况。结果 两组患者在手术时间、术中出血量、胸腔引流量、术后住院时间方面的比较具有均衡性(P>0.05);研究组患者的胃肠减压量高于对照组,组间比较具有明显差异(P<0.05)。治疗前,两组患者在肺功能指标方面的比较显示出均衡可比性(P>0.05);治疗之后,研究组FVC%、FEV1%、MVV%指标均处在高于对照组的水平,组间比较具有明显差异(P<0.05)。术前两组患者在生活质量评分方面的比较无统计学意义(P>0.05);术后研究组生活质量评分高于对照组,组间比较具有明显差异(P<0.05)。研究组患者并发症发生率(7.89%)也明显处于低于对照组(26.32%),组间比较具有明显差异(P<0.05)。结论 食管癌患者的手术切除治疗中应用细管状胃食管吻合术,可对患者肺部功能指标进行有效改善,同时可有效降低胃肠压力,降低并发症发生风险,促进术后患者生活质量有效提升。Objective To analyze the effects of fine tube gastroesophageal anastomosis on lung function,quality of life and complications in patients with esophageal cancer.Methods In a retrospective case-control study,76 patients with esopha-geal cancer admitted to the First Affiliated Hospital of Zhengzhou University from January 2020 to September 2022 were select-ed.According to different surgical methods,38 patients treated with crude tubular gastroesophageal anastomosis were selected as the control group,and 38 patients treated with fine tubular gastroesophageal anastomosis were selected as the study group,and the treatment situation of the two groups was compared.Results There was a balance between the two groups in terms of operation time,intraoperative blood loss,thoracic drainage volume and postoperative hospital stay(P>0.05).The gastro-intestinal decompression volume of the study group was higher than that of the control group,and there was a significant differ-ence between the groups(P<0.05).Before treatment,the comparison of lung function indexes between the two groups showed equilibrium(P>0.05);After treatment,the indexes of FVC%,FEV1%and MVV%in the study group were higher than those in the control group,and there were significant differences between the groups(P<0.05).Before surgery,the comparison of quality of life scores between the two groups showed a balance(P>0.05);After surgery,the quality of life score of the study group was higher than that of the control group,and there was a significant difference between the groups(P<0.05).The complication rate of the study group was also significantly lower than that of the control group,and there was a significant difference between the groups(P<0.05).Conclusion The application of fine tubular gastroesophageal anasto-mosis in the surgical resection of patients with esophageal cancer can effectively improve the pulmonary function indexes of pa-tients,effectively reduce the gastrointestinal pressure,reduce the risk of complications,and effectively improve the postope
