摘 要:目的:探究超声联合微小RNA-302(miR-302)、微小RNA-322(miR-322)表达在产前筛查胎儿神经管畸形(NTDs)临床价值。方法:回顾性收集2020年6月-2022年6月本院产前检查孕妇222例,根据分娩或引产确诊为NTDs为NTDs组(114例),未发生NTDs 108例为对照组。实时荧光定量PCR(qRT-PCR)法检测miR-302、miR-322表达水平;Pearson法分析miR-302、miR-322的相关性;绘制受试者工作特征(ROC)曲线分析miR-302、miR-322对胎儿产前NTDs的诊断价值。结果:NTDs组miR-302(1.42±0.35)高于对照组(1.01±0.13),miR-322(0.78±0.19)低于对照组(1.02±0.11)(均P<0.05),NTDs患者血清miR-302与miR-322表达呈负相关(r=-0.399,P<0.001),二者联合产前筛查胎儿NTDs的曲线下面积(AUC)(0.951)高于miR-302(0.870)和miR-322(0.907)单独诊断(P<0.001),二者联合超声产前诊断胎儿NTDs的敏感度(92.4%)高于超声(62.3%)、miR-302(75.4%)、miR-322(80.5%)单独诊断(P<0.05)。结论:胎儿NTDs孕妇血清miR-302异常高表达、miR-322异常低表达,超声联合miR-302、miR-322检测用于产前筛查胎儿NTDs有较高的诊断敏感性。Objective:To explore the clinical value of ultrasound results combined with the expressions of microRNA-302(miR-302)and microRNA-322(miR-322)of pregnant women for screening their fetal prenatal neural tube abnormalities(NTDs).Methods:222pregnant women who underwent prenatal examination in hospital from June 2020to June 2022were collected retrospectively.These women were divided 114cases with prenatal NTDs were included in group A and 108cases without prenatal NTDs were included in group B.The expression levels of miR-302and miR-322of the women in the two groups were detected by real-time fluorescent quantitative PCR(qRT-PCR).Pearson method was applied to analyze the correlation between the miR-302level of the women and their miR-322level.Receiver operator characteristic(ROC)curve was drawn to analyze the diagnostic value of the miR-302and miR-322levels of the women for their fetal prenatal NTDs.Results:The miR-302level(1.42±0.35)of the women in group A was significantly higher than that(1.01±0.13)of the women in control group B,and the miR-322level(0.78±0.19)of the women in group B was significantly lower than that(1.02±0.11)of the women in group A(all P<0.05).The expression of miR-302in serum of the women with fetal NTDs was negatively correlated with their miR-322level(r=?0.399,P<0.001).The area under the curve(AUC)of the combined miR-302and miR-322levels of the women for prenatal screening their fetal NTDs was 0.951,which was significantly higher than that(0.870)of the miR-302level or that(0.907)of the miR-322level alone(P<0.001).The sensitivity of the combined ultrasound results and the miR-302and miR-322levels of the women for prenatal screening their fetal NTDs was 92.4%,which was significantly higher than that(62.3%)of the results of ultrasound,that(75.4%)of the miR-302level,or that(80.5%)of the miR?322level alone(P<0.05).Conclusion:The miR-302in serum of the women with fetal NTDs is high expression abnormally and the miR-322is low expression abnormally.The sensitivity of the combined ultrasound result
