参附注射液对心脏骤停脑缺血再灌注损伤保护作用的研究进展

摘　　要：心脏骤停(CA)后脑缺血再灌注损伤是神经功能恢复最为关键的病理改变,影响着临床上CA患者心肺复苏(CPR)后的自主存活率及生存质量。脑缺血再灌注损伤的主要机制包括:脑内血流动力学障碍;氧自由基增多;炎症因子表达量增加;能量代谢紊乱;细胞内外离子失衡,产生钙超载,并诱导神经细胞凋亡等。参附注射液主要是由人参、附子的提取物配制的复方制剂,具有扩张冠脉,增强心肌收缩力,改善血液循环,消除氧自由基,减少血黏度等功效,临床上广泛应用于多器官保护。经科学研究证实,参附注射液对CA后脑缺血再灌注损伤具有明显保护作用,与提高脑内血液循环、减轻组织结构损伤、改善脑内能量代谢紊乱、消除氧自由基、保护细胞、调控细胞凋亡及促进神经再生等机制有关。本文综述了参附注射液在CA后缺血缺氧脑组织保护中的作用及未来前景。Cerebral ischemia-reperfusion injury after cardiac arrest(CA)is the most critical pathological change in the recovery of neurological function,which affects the autonomic survival rate and quality of life after cardiopulmonary resuscitation(CPR)in the patients with CA.The main mechanisms of cerebral ischemia-reperfusion injury include:hemodynamic disorders in the brain;the increase of oxygen radicals and the expression of inflammatory factors;energy metabolism disorders;ionic imbalance inside and outside cells causes calcium overload to induce apoptosis of nerve cells,etc.Shenfu injection is a compound preparation mainly made from the extracts of Ginseng and Aconite.It has the effects of expanding the coronary artery,enhancing myocardial contractility,improving blood circulation,eliminating oxygen free radicals and reducing blood viscosity,which is widely used to protect multiple organs in clinical.Scientific researches confirmed that Shenfu injection has obvious projective effects on cerebral ischemia-reperfusion injury after CA,which is related to improving blood circulation in brain,alleviating the damage of tissue structure,improving the disorders of energy metabolism,eliminating oxygen radicals to protect the cells,regulating cell apoptosis and prorrooting nerve regeneration,etc.In this paper,the protective effects and prospects of Shenfu injection on cerebral ischemia-reperfusion injury after CA are reviewed.
