微型营养评定法简表在老年脑卒中患者营养筛查中的应用

摘　　要：目的应用微型营养评定法简表(MNA-SF)评价老年脑卒中患者营养状况,探讨MNA-SF在老年脑卒中患者营养评估中的可行性及其相关因素。方法采用便利抽样方法,选取2018年4~10月在某三甲医院神经内科住院的320例老年脑卒中患者为研究对象,运用MNA-SF和传统营养指标测定对其营养状况进行评价。并应用日常生活活动能力量表(ADL)、简易精神状态量表(MMSE)、老年抑郁量表(GDS)、匹兹堡睡眠质量指数(PSQI)对老年脑卒中患者进行评估,分析其与营养状况的相关性。结果应用MNA-SF评估结果营养不良者占5.62%,存在营养风险者占39.38%,营养良好者占55%。18<体重指数(BMI)<23占67.9%,BMI、血红蛋白(HB)、白蛋白(ALB)、前白蛋白(PA)、红细胞计数(RBC)作为传统评价指标,患者营养不良发生率分别为2.19%、4.69%、13.13%、8.75%、11.56%。MNA-SF与患者自理能力、认知能力、抑郁状态、睡眠状态呈显著相关(P<0.05)。结论老年脑卒中患者营养不良及营养风险率高,临床中应对老年脑卒中患者进行营养筛查,为临床医生选择合理的营养支持方式提供依据。Objective To evaluate the nutritional status of elderly stroke patients using the Mini Nutritional Assessment Scale-Short Form(MNA-SF),and to explore its feasibility and influencing factors.Methods A total of 320 elderly stroke patients hospitalized in the Department of Neurology of a tertiary hospital were selected by convenience sampling method from April to October 2018.Their nutritional status was evaluated by the MNA-SF and conventional nutritional index measurement.Activities of Daily Life(ADL)Scale,the Mini-Mental State Examination(MMSE),the Geriatric Depression Scale(GDS)and the Pittsburgh Sleep Quality Index(PSQI)were assessed in elderly stroke patients,and their correlation with nutritional status was identified.Results The MNA-SF assessment showed that 5.62%of recruited elderly stroke patients were malnourished,39.38%had nutritional risk and 55%had good nutrition.There were 67.9%of elderly stroke patients with the body mass index(BMI)of 18~23kg/m 2.Using BMI,HB(hemoglobin),ALB(albumin),PA(prealbumin),and red blood cell count(RBC)as the conventional evaluation indexes,the incidence of malnutrition was 2.19%,4.69%,13.13%,8.75%and 11.56%,respectively.The MNA-SF score was significantly correlated with the self-care ability,cognition ability,depression state and sleep state(P<0.05).Conclusion Elderly stroke patients have a high rate of malnutrition and a high risk of malnutrition.Therefore,nutritional screening should be carried out in elderly stroke patients in clinical practice,thus providing a basis for clinicians to choose reasonable nutritional support methods.
