摘　　要：目的调查分析健康体检人群中幽门螺杆菌(helicobacter pylori,H.pylori)感染率,并探讨H.pylori感染与体重指数(body mass index,BMI)的相关性,为H.pylori感染和超重、肥胖人群的健康管理提供指导依据。方法选取2020年1月至2021年12月在河北省人民医院体检中心接受健康检查的24065例受检者,用13 C尿素呼吸试验检测H.pylori感染情况,通过多因素logistic回归分析H.pylori感染与BMI水平之间的相关性。结果H.pylori阳性与阴性受检者比较体重、腰围、收缩压、舒张压、血清丙氨酸氨基转移酶、γ-谷氨酰基转移酶、三酰甘油、总胆固醇、低密度脂蛋白胆固醇、血糖、尿酸、肌酐水平升高,而血清高密度脂蛋白胆固醇和白蛋白水平降低,差异均有统计学意义(P<0.05)。超重、肥胖人群的H.pylori感染率高于体质量正常和偏低人群,通过多因素logistic回归分析结果均显示,H.pylori感染与BMI水平呈正相关关系。结论H.pylori感染与超重/肥胖关联呈现正相关,而高BMI水平与H.pylori感染风险的增加相关。Objective To investigate the prevalence of Helicobacter pylori(Hp)infection in healthy volunteers receiving physical examination,and to investigate the correlation between Hp infection and body mass index(BMI),so as to provide guidance for the health management of Hp infection,as well as overweight and obese.Methods From January 2020 to December 2021,totally 24065 subjects receiving physical examinations in the Health Examination Centre of Hebei General Hospital were selected for testing Hp infection using the 13C urea breath test.The correlation between Hp infection and body mass index(BMI)was analyzed by the multivariable logistic regression.Results Compared with those of Hp-negative subjects,Hp-positive subjects presented significantly higher body weight,waist circumference,systolic blood pressure,diastolic blood pressure,serum alanine transferase,γ-glutamyltransferase,triglyceride,total cholesterol,low-density lipoprotein cholesterol,blood sugar,uric acid and creatinine levels,but significantly lower high-density lipoprotein cholesterol and albumin(P<0.05).The prevalence of Hp infection was significantly higher in overweight and obese people than that of people with normal or low BMI.The multivariable logistic regression analysis showed a positive correlation between Hp infection and BMI.Conclusion Hp infection is positively associated with overweight/obesity,and a high BMI indicates an increased risk of Hp infection.
