原发性腹膜后肿瘤细胞减灭术后复发因素分析

摘　　要：目的探讨原发性腹膜后肿瘤(PRT)细胞减灭术后复发的因素。方法回顾性分析2018-01—2022-01郑州大学第二附属医院普外科行细胞减灭术治疗的88例PRT患者的临床资料。男51例,女37例;年龄(52.37±5.47)岁(范围:40~69岁)。采用Logistic回归进行多因素分析确定PRT细胞减灭术后复发的影响因素。结果全部患者均于细胞减灭术后随访1a,复发22例。Logistic回归分析结果显示,肿瘤多发、肿瘤形态不规则、肿瘤类型为恶性、肿瘤直径≥5 cm为PRT细胞减灭术后复发的影响因素(OR>1,P<0.05)。结论肿瘤多发、肿瘤形态不规则、肿瘤类型为恶性、肿瘤直径≥5 cm为PRT细胞减灭术后复发的影响因素。明确术后复发的影响因素有助于指导临床医师对术后复发的高危患者进行筛选,并采取针对性防治策略,改善患者预后。Objective To explore the recurrence factors of primary retroperitoneal tumor(PRT)after cytoreductive surgery.Methods A retrospective analysis of 88 patients with primary retroperitoneal tumor who received cytoreductive surgery in General Surgery of the the Second Affiliated Hospital of Zhengzhou University from January 2018 to December 2021.Totally 51 male and 37 female;Age(52.37±5.47)years(range:40~69 years).Using multifactorial Logistic regression model to analyze the the influence factors of PRT recurrence after cytoreductive surgery.Results All patients were follow-up of 1 year after cytoreductive surgery,22 cases were recurred.Logistic regression analysis results show that the multiple tumor,tumor irregular shape,malignant tumor,tumor diameter greater than or equal to 5 cm are the influence factors of PRT recurrence after cytoreductive surgery(OR>1,P<0.05).Conclusion multiple tumor,tumor shape is irregular,malignant tumor,tumor diameter greater than or equal to 5 cm are the influence factors of PRT recurrence after cytoreductive surgery.Make clear the influence factors of postoperative recurrence help clinicians filtrate high-risk patients of postoperative recurrence and adopt corresponding prevention and treatment strategy,improving the prognosis of patients.
