摘　　要：目的探讨不同偏颌畸形患者正颌手术方法选择,为后续正颌治疗手术方案的设计提供参考。方法回顾性分析2020-01—2023-03于郑州大学第一附属医院口腔颌面外科行正颌手术的23例偏颌畸形患者的临床资料。男8例,女15例,年龄(29.12±11.03)岁(范围:18~40岁)。根据不同偏颌畸形,酌情采用经口内切口的下颌升支矢状劈开截骨术、经口内切口的上颌LeFortⅠ型截骨术、髁突成形术及颏成形术。结果23例患者术后随访6~12个月,影像检查测得颏中线偏离距离均在2 mm以内,偏颌畸形得到明显改善,面部轮廓基本对称,患者均对自己面型的改观较为满意,偏颌症状无复发迹象。颏下点至面中线的距离较术前明显减小,差异有统计学意义(P<0.01)。结论借助三维模拟技术,并在正畸医师协同下进行术前分析、制作颌板及手术截骨导板,制定适合患者的个性化手术方案,结合精准术中操作,可最大程度降低手术损伤并获得理想的手术效果。Objective To explore the selection of orthognathic surgery methods in patients with different partial jaw deformities,and to provide reference for the design of subsequent orthognathic treatment schemes.Methods A total of 23 patients with partial jaw deformity who admitted to the Department of Oral and Maxillofacial Surgery of the First Affiliated Hospital of Zhengzhou University from January 2020 to March 2023 were studied as the research subjects,and the general information,surgical methods,curative effects and surgical experience of the patients were summarized.Results Patients with different types of hemiabular deformities were treated with orthognathic surgery and orthodontic treatment according to their own conditions,and all obtained satisfactory results.Conclusion With the help of 3D simulation technology and the collaboration of orthodontists,preoperative analysis,jaw plate and surgical osteotomy guide plate are made,and a personalized surgical plan suitable for patients is formulated,combined with precise intraoperative operation,so as to minimize surgical damage and obtain ideal surgical results.
