摘  要:目的探讨机器人联合显微镜辅助MIS-TLIF术在腰椎退变性疾病中的临床应用价值。方法回顾性分析2019年9月~2020年9月本院收治的96例腰椎退变性疾病患者临床资料,根据手术方式分为观察组(47例)和对照组(49例),观察组采用机器人联合显微镜辅助MIS-TLIF术治疗,对照组采用显微镜辅助MIS-TLIF术治疗。比较两组患者的相关指标。结果观察组患者术中出血量、术中透视次数、术后引流量明显少于对照组,术后下地时间、术后住院时间明显短于对照组,治疗优良率及椎弓根螺钉置钉准确率明显高于对照组(P<0.05)。两组患者术后3 d、术后1年的腰痛VAS评分、下肢痛VAS评分、ODI评分明显低于术前,Cobb角明显大于术前(P<0.05);观察组患者术后3 d的腰痛VAS评分、下肢痛VAS评分、ODI评分明显低于对照组,Cobb角明显大于对照组(P<0.05);两组术前及术后1年的上述指标差异无统计学意义(P>0.05);随机行走模型结果显示,观察组患者上述指标改善情况明显优于对照组(P<0.05)。两组患者术后并发症发生率无明显差异(P>0.05),且无椎弓根螺钉和椎间融合器断裂、移位情况的发生。结论机器人联合显微镜辅助MIS-TLIF术能够提高腰椎退变性疾病患者的疗效及椎弓根螺钉置入的准确性,安全可靠,值得临床推广应用。Objective To explore the clinical value of robot combined with microscope-assisted MIS-TLIF in lumbar degenerative diseasesvia comparison with microscope-assisted minimally invasive transforaminal interbody fusion(MIS-TLIF).Methods A retrospective analysis was performed on the clinical data of 96 patients with degenerative lumbar spine diseases admitted to our hospital from September 2019 to September 2020.According to the operation method,they were divided into the observation group(47 cases)and the control group(49 cases).The observation group used robotcombined with microscope-assisted MIS-TLIF surgery,and the control group was treated with microscope-assisted MIS-TLIF surgery.The relevant indicators of the two groupswere compared.Results In the observation group,intraoperative blood loss,intraoperative fluoroscopy,and postoperative drainage volume were significantly lower than those of the control group,and the postoperative time to field and postoperative hospital stay were significantly shorter than those of the control group.The excellent rate of treatment and the accuracy of pedicle screw placement were significantly higher than those of the control group(P<0.05);the VAS scores of low back pain,the VAS scores of lower limb pain,and the ODI scores of the two groups of patients were significantly lower than those of the preoperative 3 d and 1 year after the operation,and the Cobb angle was significantly greater than that of the preoperative(P<0.05).The low back pain VAS score,lower limb pain VAS score,and ODI score of the observation group were significantly lower than those of the control group 3 days after operation,and the Cobb angle was significantly greater than that of the control group(P<0.05).There was no statistically significant difference in the above indicators between the two groups before and 1 year after operation(P>0.05).The results of the random walking model showed that the improvement of the above indicators in the observation group was markedly better than that in the control group(P<0.0
