玻璃陶瓷全瓷冠与高透氧化锆全瓷冠在前牙美学修复中的应用比较

摘　　要：目的:分析玻璃陶瓷全瓷冠与高透氧化锆全瓷冠在前牙美学修复中的应用效果。方法:选取2018年6月-2020年6月行前牙美容修复治疗的90例患者,根据修复体不同分为观察组(n=45,修复体63个)和对照组(n=45,修复体56个)。对照组采用IPS e.max Press玻璃陶瓷全瓷冠修复,观察组采用威兰德臻瓷(Zenostar Zirconia)高透氧化锆全瓷冠修复。比较两组修复体随访12个月时的不良事件发生情况及美观度,并记录两组患者治疗满意度。结果:随访12个月时,两组均获得良好治疗效果,观察组修复体不良事件发生率为9.52%(6/63),对照组为7.14%(4/56),组间比较差异无统计学意义(P>0.05),两组治疗1~6个月、7~12个月,修复体出现不良事件例数及不良事件类型比较差异无统计学意义(P>0.05);随访12个月时,观察组未出现不良事件修复体的颜色匹配评分显著低于对照组(P<0.05),两组未出现不良事件修复体的形态匹配、继发龋、敏感、边缘适应性评分比较差异无统计学意义(P>0.05);随访12个月时,观察组患者的修复体颜色满意度显著低于对照组,而两组患者的修复体形态及牙周组织健康状态满意度比较差异无统计学意义(P>0.05)。结论:玻璃陶瓷全瓷冠及高透氧化锆全瓷冠均可有效修复前牙缺损,两者在形态匹配、边缘适应性、牙龈敏感性等方面疗效相当,但前者的颜色匹配度更佳,临床应根据患者实际情况选择治疗方式。Objective To analyze the application effects of glass-ceramic all-ceramic crown and high-permeability zirconia all-ceramic crown in the aesthetic restoration of anterior teeth.Methods 90 patients who underwent anterior teeth aesthetic restoration were selected between June 2018 and June 2020,and they were divided into observation group(n=45,63 restorations)and control group(n=45,56 restorations).The control group was repaired with IPS e.max Press glass-ceramic all-ceramic crown,while the observation group was repaired with Zenostar Zirconia high-permeability zirconia all-ceramic crown.The occurrence of adverse events and aesthetic degree of the two groups of restorations were compared at 12 months of follow-up,and the treatment satisfaction was recorded in the two groups.Results At 12 months of follow-up,both groups achieved good treatment effects,and the incidence rate of restoration adverse events was 9.52%(6/63)in observation group and that in control group was 7.14%(4/56)(P>0.05).During the treatment periods of 1-6 months and 7-12 months,there was no significant difference in the number and type of adverse events between the two groups(P>0.05).At 12 months of follow-up,the score of color matching of restorations without adverse events in observation group was significantly lower than that in control group(P<0.05),but there were no statistical differences in the scores of shape matching,secondary caries,sensitivity and edge adaptability of restorations without adverse events between the two groups(P>0.05).At 12 months of follow-up,the color satisfaction degree of restoration was significantly lower in observation group than that in control group,but there were no statistical significance in the satisfaction degrees of restoration shape and periodontal health status between the two groups(P>0.05).Conclusion Both glass-ceramic all-ceramic crown and high-permeability zirconia all-ceramic crown can effectively repair anterior teeth defects.The two have similar efficacy in terms of shape matching,edge adaptability
