2012—2021年重庆市石柱县恶性肿瘤死亡率及死亡疾病负担趋势分析

摘　　要：目的了解重庆市石柱县恶性肿瘤死亡率、早死寿命损失情况及趋势,为开展恶性肿瘤防治提供建议。方法恶性肿瘤死亡个案资料来源于2012—2021年重庆市石柱县死因监测系统,采用SPSS 25.0软件统计分析死亡率、标化死亡率、早死寿命损失年(years of life lost,YLL)、平均寿命损失年(average years of life loss,AYLL),男性与女性死亡率的比较采用χ2检验,率的趋势分析采用年度变化百分比(annual percent change,APC)。结果重庆市石柱县恶性肿瘤粗死亡率由2012年的151.79/10万上升至2021年的186.38/10万,APC为3.977%(P<0.05)。2012年石柱县恶性肿瘤标化死亡率为99.58/10万,2021年为91.31/10万,APC为0.904%,变化趋势差异无统计学意义(t=0.772,P>0.05)。2012—2021年男性恶性肿瘤粗死亡率均大于女性。男性恶性肿瘤粗死亡率以年均4.498%上升(P<0.05),女性恶性肿瘤粗死亡率则保持相对稳定(P>0.05)。<40岁年龄组恶性肿瘤死亡率以年均7.504%下降,80岁及以上年龄组恶性肿瘤死亡率以年均5.866%上升(P<0.05)。石柱县恶性肿瘤YLL率和女性YLL率保持相对稳定(P>0.05),男性YLL率以年均2.737%上升(P<0.05);石柱县恶性肿瘤AYLL、男女性恶性肿瘤AYLL分别以年均2.078%、1.686%、2.664%下降(P<0.05)。结论重庆市石柱县恶性肿瘤死亡率呈上升趋势,死亡疾病负担较重,应进一步加强恶性肿瘤的综合防治。Objective To understand the situation and trends of malignant tumor mortality,early death and life loss in Shizhu County,and to provide suggestions for prevention and treatment of malignant tumors.Methods The data of malignant tumor death cases were collected from the Death Cause Surveillance System in Shizhu County from 2012 to 2021.SPSS25.0 software was used to statistically analyze the mortality,standardized mortality,years of life lost with premature death(YLL)and average years of life loss(AYLL).Chi square test was employed to compare the mortalities of males and females.The annual percent change(APC)was applied to analyzing the trends in the mortalities.Results The crude mortality of malignant tumors in Shizhu County increased from 151.79/100,000 in 2012 to 186.38/100,000 in 2021,with the APC being 3.977%(P<0.05).The standardized mortalities of malignant tumors in Shizhu County in 2012 and 2021 were 99.58/100,000 and 91.31/100,000 respectively,and the APC was 0.904%,showing no statistically significant difference(t=0.772,P>0.05).The crude mortalities of malignant tumors in males in 2012-2021 were all higher than those in females.The crude mortalities of malignant tumors in males increased at an annual average of 4.498%(P<0.05),while those in females remained relatively stable(P>0.05).The mortalities of malignant tumors in the group aged<40 years decreased by an annual average of 7.504%,whereas those in the group aged 80 years and above increased at an annual average of 5.866%(P<0.05).The YLL rate due to malignant tumors and the YLL rate in females in Shizhu County remained relatively stable(P>0.05),and the YLL rate in males increased at an annual average of 2.737%(P<0.05).The AYLL due to malignant tumors and the AYLLs due to malignant tumors in males and females in Shizhu County dropped by an annual average of 2.078%,1.686%and 2.664%respectively(P<0.05).Conclusion The mortality of malignant tumors in Shizhu County in 2012-2021 showed an upward trend,and the disease burden caused by deaths from malignancies
