摘　　要：目的识别不同组织学分级原发性软骨肉瘤患者的临床、影像及病理组织学特征,以期提高其诊断与鉴别诊断的水平。方法回顾性分析2015年1月~2022年6月山西医科大学第二医院经手术病理证实的67例原发性软骨肉瘤患者的资料,按组织学分级进行分组,分析其临床、影像及病理组织学特征。结果67例原发性软骨肉瘤患者肿瘤发病部位、瘤体体积、软组织肿块、骨质破坏、骨髓或软组织水肿及ki-67表达情况在低级别与中高级别组间差异有统计学意义(P均<0.05);Pearson相关性分析结果显示,ki-67阳性细胞百分比与异柠檬酸脱氢酶1(isocitrate dehydrogenase 1),IDH1基因突变情况呈正相关(r=0.35,P=0.039)。结论不同级别原发性软骨肉瘤患者有不同的临床、影像及病理组织学特征,可为原发性软骨肉瘤术前准确分级提供参考依据。Objective To identify the clinical,imaging and histopathological features of patients with different grades of primary chondrosarcoma,in order to improve the level of diagnosis and differential diagnosis.Methods The data of 67 patients with primary chondrosarcoma confirmed by surgery and pathology in the Second Hospital of Shanxi Medical University from January 2015 to June 2022 were retrospectively analyzed.The patients were grouped according to histological grade,and their clinical,imaging and histopathological features were analyzed.Results There were significant differences in tumor location,tumor volume,soft tissue mass,bone destruction,bone marrow or soft tissue edema,and ki-67 expression in 67 patients with primary chondrosarcoma between the low-grade and middle-high-grade groups(P<0.05);Pearson correlation analysis showed that the percentage of ki-67 positive cells was positively correlated with IDH1gene mutation(r=0.35,P=0.039).Conclusion Patients with different grades of primary chondrosarcoma have different clinical,imaging and histopathological features,which can provide a reference for accurate preoperative grading of primary chondrosarcoma.
