摘 要:目的探讨ZOOMit IVIM和T_(1)mapping的定量参数在鉴别肺良恶性病变中的作用并评估它们的诊断效能。方法选取76例诊断为“肺占位性病变”的患者行肺MRI检查,良性11例(良性组),肺癌65例(肺癌组),测量ADC、D、D~*、f和T_(1)等参数,对这些参数进行分析,并使用受试者工作特征曲线评估诊断效能。结果良性组与肺癌组之间对比分析,良性组的ADC值、D值、f值和T_(1)值均大于肺癌组(ADC值P<0.001;D值P<0.001;f值P=0.004;T_(1)值P=0.026)。ROC曲线分析结果显示ADC值的AUC最大,诊断效能最好(ADC值AUC=0.9552;D值AUC=0.9483;f值AUC=0.7769;T_(1)值AUC=0.710)。而D~*值在两组之间差异无统计学意义(P=0.430)。结论ADC、D、f和T_(1)有助于鉴别肺的良恶性病变,ADC值的诊断效能最高。Objective To explore the role of quantitative parameters of ZOOMit IVIM and T_(1) mapping in identifying benign and malignant pulmonary lesions and to assess their diagnostic efficacy.Methods 76 patients diagnosed with"pulmonary occupying lesions"underwent lung MRI examination,with 11 cases diagnosed as benign and 65 cases diagnosed as lung cancer.ADC,D,D*,f,and T_(1) parameters were measured and analyzed,and the diagnostic performance was evaluated using receiver operating characteristic curves.Results The comparative analysis between the benign and lung cancer groups showed that the ADC,D,f,and T_(1) values of the benign group were all higher than those of the lung cancer group(ADC P<0.001;D P<0.001;f P=0.004;T_(1) P=0.026).The results of ROC analysis showed that the ADC value had the best diagnostic efficacy(ADC AUC=0.9552;D AUC=0.9483;f AUC=0.7769;T_(1) AUC=0.710).And the D*values were not statistically different between the two groups(P=0.430).Conclusion ADC,D,f,and T_(1) could help identify benign and malignant pulmonary lesions,with ADC values having the highest diagnostic efficacy.
